摘  要:目的评价三维时间飞跃法磁共振血管成像(3D-TOF-MRA)联合三维双激发平衡式稳态自由进动(3D-FIESTA-C)序列在诊断血管压迫性面肌痉挛中的应用价值。方法 24例经微血管减压术证实为血管压迫性面肌痉挛患者,均行3D-TOF-MRA及3D-FIESTA-C序列扫描,以术中所见为金标准,评价二种序列结合多平面重建及图像融合技术对面肌痉挛的诊断效能。结果术前3D-TOF-MRA、3D-FIESTA-C及3D-TOF-MRA+3D-FIESTA-C对责任血管的检出阳性率分别为62.5%(15/24)、66.7%(16/24)、87.5%(21/24),其中3D-TOF-MRA+3D-FIESTA-C检出阳性率高于3D-TOF-MRA(P<0.05)。术前3D-TOF-MRA、3D-FIESTA-C及3D-TOF-MRA+3D-FIESTA-C对责任动脉种类的显示准确率分别为71.4%(15/21)、76.2%(16/21)、100.0%(21/21),3D-TOF-MRA+3D-FIESTA-C检测的准确率均高于3D-TOF-MRA或3D-FIESTA-C(均P<0.05)。结论结合多平面重建及图像融合技术,3D-TOF-MRA联合3D-FIESTA-C序列能准确评估血管压迫性面肌痉挛患者的责任血管性质和神经血管压迫征象,对外科微血管减压术术前评估有重要意义。Objective To investigate the value of three-dimensional time-of-flight magnetic resonance angiography(3D-TOF-MRA)combined with three-dimensional fast imaging employing steady state acquisition with cycled phases(3D-FIESTA-C)sequences applied to diagnosis of hemifacial spasm due to vascular compression.Methods Twenty-four patients diagnosed as hemifacial spasm due to vascular compression by microvascular decompression underwent the scanning with 3D-TOF-MRA and 3D-FIESTA-C sequences.The surgical result was taken as golden standard,the diagnostic efficiency of both sequences combined with multiplanar reconstruction and image fusion technique for hemifacial spasm was assessed.Results Before operation,the positive detection rates of 3D-TOF-MRA,3D-FIESTA-C and 3D-TOF-MRA+3D-FIESTA-C for offending vessels were 62.5%(15/24),66.7%(16/24)and 87.5%(21/24)respectively,and the rate of 3D-TOF-MRA+3D-FIESTA-C was higher than that of 3D-TOF-MRA(P<0.05).Before operation,the accurate rate of 3D-TOF-MRA,3D-FIESTA-C and 3D-TOF-MRA+3D-FIESTA-C for revealing the type of offending arteries were 71.4%(15/21),76.2%(16/21),and 100.0%(21/21)respectively,and the rate of 3D-TOF-MRA+3D-FIESTA-C was higher than that of 3D-TOF-MRA or 3D-FIESTA-C(all P<0.05).Conclusion On the basis of multiplanar reconstruction and image fusion technique,3D-TOF-MRA combined with 3D-FIESTA-C sequences can accurately assess the type of offending vessels and the neurovascular compression signs for patients with hemifacial spasm due to vascular compression,which is highly significant for the preoperative evaluation of surgical microvascular decompression.
