ROC曲线评价血清鳞状细胞癌抗原诊断慢性肾功能衰竭的价值

摘　　要：目的:评价鳞状细胞癌抗原(squamous cell carcinoma antigen,SCCA)对慢性肾功能衰竭(chronic renal failure,CRF)的诊断价值。方法 :回顾性分析2014年1—12月我院所有进行SCCA检测病例,以1.5 ng/mL为诊断分界点,计算各疾病SCCA异常者所占比例;分别比较健康体检者与CRF肾功能不全代偿期、肾功能不全失代偿期、肾功能衰竭期和尿毒症期患者血清SCCA浓度水平;用受试者工作特征曲线(receiver operating characteristic curve,ROC曲线)分析SCCA对CRF各期诊断曲线下面积(area under the curve,AUC)、敏感性和特异性。结果:CRF患者SCCA异常比例最高,为68.3%;CRF各期与健康体检组SCCA质量浓度水平差异均有统计学意义(P<0.05),肾功能不全代偿期与CRF其他3期SCCA质量浓度水平差异均有统计学意义(P<0.05);SCCA对CRF各期的诊断AUC分别为0.716±0.030、0.922±0.014、0.940±0.013和0.941±0.013,敏感性分别为64.81%、88.48%、83.93%和88.44%,特异性分别为70.24%、81.55%、92.26%和88.69%。结论 :SCCA对CRF具有较高的辅助诊断价值,值得在临床推广应用。Objective:To study the diagnostic value of squamous cell carcinoma antigen(SCCA)for chronic renal failure(CRF).Methods:Retrospective analysis of all test cases of SCCA in 2014 of our hospital,the cut-off value of SCCA for diagnosis is 1.5 ng/mL,then calculation the proportion with abnormal SCCA of each kind of disease.Comparing SCCA concentration levels of healthy people with CRF patients,including renal insufficiency compensatory period,renal insufficiency decompensated period,renal failure period and uremia period,then statistical analysis whether the differences between SCCA concentrations are significant;diagnosis of the area under the curve(AUC),the sensitivity and specificity for SCCA to diagnosis of CRF by receiver operating characteristic curve(ROC curve).Results:CRF patients have the highest proportion with abnormal SCCA,which is 68.3%;the difference of SCCA concentrations between healthy people and CRF patients is significant(P<0.05),between renal insufficiency compensatory period and other three periods is also significant(P<0.05);AUC for SCCA to diagnosis of renal insufficiency decompensated period,renal failure period and uremia period were 0.716±0.030,0.922±0.014,0.940±0.013 and 0.941±0.013,respectively.The sensitivity were 64.81%,88.48%,83.93%and 88.44%,respectively.The specificity were 70.24%,81.55%,92.26%and 81.55%respectively.Conclusion:SCCA has high diagnostic value for CRF,which is worth popularizing in clinical application.
