摘 要:目的:探讨新生儿高胆红素血症与甲状腺功能指标的相关性。方法 :收集南通大学附属医院收治确诊为高胆红素血症的足月新生儿血清样本(n=130),同时选取无黄染的健康新生儿血清标本作为对照组(n=40),采用化学发光法分别检测血清标本的游离三碘甲状原氨酸(free triiodothyronine,FT3)、游离甲状腺素(free thyroxine index,FT4)、总三碘甲状腺原氨酸(total triiodothyronice,TT3)、总四碘甲状腺原氨酸(total thyroxine,TT4)、促甲状腺激素(thyroid stimulating hormone,TSH)含量,比较两组甲状腺功能指标的差异。同时,利用全自动生化仪检测血清总胆红素(total bilirubin,TBIL)含量,分析血清TBIL指标与甲状腺功能指标的相关性。结果:与对照组相比,高胆红素血症患儿FT3明显降低[(4.50±0.66)pmol/L vs(5.29±0.89)pmol/L,P<0.05],TT3含量也明显减低[(1.56±0.33)nmol/L vs(2.05±0.53)nmol/L,P<0.05]。高胆红素血症患儿血清TBIL水平明显高于对照组,且与FT3水平、TT3水平呈负相关。而高胆红素血症患儿FT4(18.09±3.65)pmol/L、TT4(120.33±24.53)nmol/L、TSH(5.08±1.29)m IU/L含量与对照组差异无统计学意义,且与血清TBIL水平无明显相关性。结论:随着血清TBIL的增加,高胆红素血症患儿血清FT3、TT3水平显著下降,FT3、TT3水平有利于监测高胆红素血症患儿的甲状腺功能,有利于降低患儿智障发生率,提高患儿预后生活质量。Objective:To investigate the relationship between neonatal hyperbilirubinemia and thyroid function.Methods:Nasal neonatal serum samples(n=130)with hyperbilirubinemia were collected from Nantong University Hospital,and healthy neonates without xanthochromia were selected as control group(n=40).Chemiluminescence method was used to detect the content of free triiodothyronine(FT3),free thyroxine index(FT4),total triiodothyronice(TT3),total thyroxine(TT4),thyroid stimulating hormone(TSH)and compared with the healthy control group.In addition,serum total bilirubin(TBIL)level were detected by automated biochemical instrument to assay the correlation between TBIL and FT3,FT4,TT3,TT4 and TSH.Results:Compared with the control,the FT3 of neonates with hyperbilirubinemia was significantly reduced[(4.50±0.66)pmol/L vs(5.29±0.89)pmol/L,P<0.05],TT3 content was also significantly reduced[(1.56±0.33)nmol/L vs(2.05±0.53)nmol/L,P<0.05].The level of TBIL of neonates with hyperbilirubinemia was significantly higher than control group,the difference between groups was statistically significant(P<0.05).And there was a negative correlation between the level of FT3, TT3.However,the FT4(18.09±3.65)pmol/L,TT4(120.33±24.53)nmol/L and TSH(5.08±1.29)mIU/L had no significant difference with the normal control group,and there was no significant correlation between serum FT4,TT4 or TSH level and TBIL level.Conclusion:With the increase of TBIL,the levels of serum FT3 and TT3 were significantly decreased in neonatal hyperbilirubinemia.Monitoring the levels of FT3 and TT3 are beneficial to detect thyroid function of neonatal hyperbilirubinemia,which is useful for reducing incidence of mental retardation in children and improve the quality of life.
