腰椎后路内置物植入术不同镇痛方法的效果比较

摘　　要：目的探讨腰椎后路内置物植入术缝合前术区局部应用镇痛药的镇痛效果。方法将笔者所在科2013年11月—2015年5月接受腰椎后路内置物植入术的486例患者,随机分为观察组、对照组1、对照组2,观察组于缝合前术区局部应用镇痛药(利多卡因+罗哌卡因)、对照组1于术后留置静脉镇痛泵(芬太尼+地佐辛)、对照组2于术后疼痛时肌肉注射曲马朵注射液;采用视觉模拟法评价患者疼痛干预效果。结果观察组使用镇痛药后于各个时间点测得VAS得分均低于其他两组(P<0.05);重复使用镇痛药物例次,观察组少于其他两组(P<0.05);观察组无药物不良反应发生。结论观察组方法应用简便,安全性高,镇痛效果明显,未发现明显的不良反应,有临床推广意义。Objective To investigate the effect of local application of analgesic method before suturing incision in the posterior lumbar interbody fusion.Methods Authors retrospectively analyzed a series of 486 patients who accepted the posterior lumbar interbody fusion and were recruited to this study from Nov.2013 to May 2015yr.All the patients were divided into observation group,control group I and control group II according to the principle of randomization.Observation group were used the analgesic before suture the incision,crotrol group I with auto-controlled analgesic pump and the control group II were used Tramadol Hydrochloride Injection when the patient could not tolerate the pain.The visual analog scale(VAS)score was used to evaluate the analgesic effect.Results The VAS score of observation group were all belew control group I and control group II at every measurement time point(P<0.05),the reuse of the analgesics was all less than control group I and control group II(P<0.05),there was no complications that poisonous side effect of the analgesic medicine in observation group.Conclusions The analgesic methods used in observation group is a simpler,safer and more effective method,is an valuable analgesic method in clinical practice for the security.
