基线CD4^+T细胞值与艾滋病抗病毒治疗后的死亡病例分析

摘　　要：目的:分析大理市接受抗病毒治疗的HIV/AIDS患者基线CD4^+细胞水平与病死情况之间的关系,为提高抗病毒治疗效果提供参考依据。方法:从国家系统中收集病例进行分析。结果:接受抗病毒治疗的1 303例HIV/AIDS患者中,累计死亡110例,病死率为8.44%。基线CD4^+细胞<50个/mm^3的患者病死率为17.36%,基线CD4^+细胞>200个/mm^3的患者病死率为4.03%。国家艾滋病免费抗病毒治疗标准调整前后的病死率也由最初的18.22%逐步降至3.57%。死亡病例中,19.09%病例的基线CD4^+细胞<50个/mm^3,其中87.5%死于艾滋病相关疾病;23.64%病例的基线CD4^+细胞>200个/mm^3,28.57%是死于艾滋病相关疾病。结论:早期及时地开始抗病毒治疗是降低艾滋病病死率的关键。Objective:To analyze the relationship between baseline level of CD4+T cell count and mortality status among HIV/AIDS patients being given highly active antiretroviral therapy in Dali city,and to provide reference for improving the effectiveness of AIDS antiviral treatment.Methods:HIV/AIDS patients from the National AIDS Comprehensive Prevention and Treatment System:the Management of Antivirus Treatment Database were collected and analyzed by descriptive epidemiology.Results:110 patients died among 1 303 HIV/AIDS patients.The fatality rate was 8.44%.When starting treatment,mortality rate of those with CD4+cell count<50/mm3 was 17.36%and the mortality rate of those with CD4+cell count>200/mm3 was 4.03%.The mortality rate decreased from 18.22%to 3.57%because the modified antiviral treatment standard has expanded the scope of treatment in china.In death cases,19.09%of the patients'baseline CD4+cell count<50/mm3,87.5%of deaths died of AIDS related diseases.23.64%of the patients'baseline CD4+cell count>200/mm3,28.57%of deaths died of AIDS related diseases.Conclusion:Earlier initiation of highly active antiretroviral therapy is the key to reducing AIDS mortality.
