平肝降压配方颗粒联合自我效能管理对妊娠高血压患者心理应激反应及妊娠结局的影响

摘　　要：目的探讨平肝降压配方颗粒联合自我效能管理对妊娠高血压患者心理应激反应及妊娠结局的影响。方法选择医院2015年5月至2017年1月收治的妊娠高血压孕产妇108例,采用随机数字表法分为对照组及观察组,各54例。两组孕产妇均予硫酸镁冲击治疗联合静脉滴注,同时口服平肝降压配方颗粒,每次2袋,每日3次,7 d为1个疗程,共治疗2疗程。在此基础上,观察组加用自我效能管理。结果观察组产妇总有效率为94.44%,稍高于对照组的90.74%(P>0.05);治疗后,两组孕产妇收缩压(SBP)、舒张压(DBP)、汉密尔顿抑郁及焦虑量表(HAMA及HAMD)评分均较治疗前明显下降,且观察组下降更明显(P<0.05);观察组孕产妇子痫发生率及不良妊娠结局总发生率,新生儿并发症发生率均稍低于对照组(P>0.05)。结论平肝降压配方颗粒联合自我效能管理对妊娠高血压孕产妇的临床疗效显著,可显著调节孕产妇心理应激反应,并能降低不良妊娠结局、子痫及新生儿并发症的发生率,且不良反应较轻微,不影响治疗,值得临床推广。Objective To investigate the effect of Pinggan Jiangya Dispensing Granules combined with self-efficacy management on psychological stress response and pregnancy outcomes in patients with hypertension of pregnancy.Methods Totally 180 pregnant women with hypertension of pregnancy admitted to our hospital from May 2015 to January 2017 were selected and divided into the control group and the observation group according to the random number table method,54 cases in each group.The two groups were given pulse therapy with magnesium sulfate and intravenous drip of magnesium sulfate,and given Pinggan Jiangya Dispensing Granules at the same time,2 bags/time,3 times/day,7 d for a course of treatment,the two groups were treated for 2 courses of treatment.On this basis,the observation group was given self-efficacy management.Results The total effective rate of the observation group was 94.44%,which was higher than 90.74%of the control group(P>0.05).After treatment,the scores of SBP,DBP,HAMA and HAMD in the two groups were significantly lower than those before treatment,and the decrease in the observation group was more obvious(P<0.05).The incidence rate of eclampsia,the total incidence rate of adverse pregnancy outcomes and the incidence rate of neonatal complications in the observation group were lower than those of the control group(P>0.05).Conclusion Pinggan Jiangya Dispensing Granules combined with self-efficacy management for treating patients with hypertension of pregnancy has significant effect,it can significantly regulate the psychological stress reaction of pregnant women,and reduce the adverse pregnancy outcomes and the incidence rate of eclampsia and neonatal complications,and the adverse reactions are mild.It does not affect the treatment,and it is worthy of clinical promotion.
