八髎穴电针为主治疗脊髓损伤后神经源性膀胱的临床观察

摘　　要：目的观察八髎穴电针刺激结合膀胱体表投影区神经电刺激法(即骶神经起搏法)对脊髓损伤后神经源性膀胱的治疗效果。方法将32例脊髓损伤所致神经源性膀胱患者随机分为治疗组18例和对照组14例。在基础康复治疗的基础上,治疗组进行骶神经起搏法治疗(八髎穴电针刺激法结合膀胱体表投影区神经电刺激法),对照组予以常规膀胱训练方法。比较两组治疗前后患者膀胱功能改善情况,即治疗前后残余尿量、最大膀胱容量、自主排尿次数、漏尿次数、平均控尿费用等指标。结果治疗后两组残余尿量、最大膀胱容量、自主排尿次数、漏尿次数、平均控尿费用较同组治疗前明显改善(P<0.05),两组治疗后膀胱功能指标及平均控尿费用比较差异有统计学意义(P<0.05)。结论八髎穴电针刺激结合膀胱体表投影区神经电刺激法能较好地促进脊髓损伤患者神经源性膀胱的功能恢复。Objective To observe the treatment efficacy of electroacupuncture at Baliao points[Shangliao(BL31),Ciliao(BL32),Zhongliao(BL33),Xialiao(BL34)]plus nerve stimulation at surface projection of bladder(sacral nerve pacing)in treating neurogenic bladder after spinal cord injury.Method A total of 32 patients with neurogenic bladder due to spinal cord injury were randomized into a treatment group of 18 cases and a control group of 14 cases.On the basis of fundamental rehabilitation treatment,the treatment group was given sacral nerve pacing treatment(electroacupuncture at Baliao points plus nerve stimulation at surface projection of bladder),and the control group was intervened by conventional bladder training method.The changes of bladder function were compared in the two groups before and after the treatment,such as the residual urine volume,maximum cystometric capacity,number of normal urination and urine leakage,and the average cost of urinary control.Result The residual urine volumes,maximum cystometric capacities,numbers of normal urination,numbers of urine leakage and the average costs of urinary control were significantly improved in the two groups after the treatment(P<0.05).There were significantly statistical differences in comparing the bladder function indexes and the average costs of urinary control between the two groups after the treatment(P<0.05).Conclusion Electroacupuncture at Baliao points plus nerve stimulation at surface projection of bladder(sacral nerve pacing)can better promote the recovery of bladder function in patients with neurogenic bladder after spinal cord injury.
