Effect of “Secondary-primary Point Combination” Electroacupuncture on Cardiac Function and Apoptosis Index in a Rat Model of Myocardial Ischemia

摘  要:目的通过"标本配穴"电针对针刺心肌缺血大鼠治疗前后心脏功能和细胞凋亡指数影响的实验研究,观察治疗疗效,从而探讨针刺预处理对心肌的保护机制,为临床治疗该病提供科学的方法。方法 40只大鼠按随机数字表分成正常组、模型组、内关组、标配组。采取连续14 d腹部皮下注射异丙肾上腺素造模。正常组、模型组大鼠只抓取固定,不针刺治疗。内关组电针双侧内关,标配组电针双侧内关、关元、双侧足三里,每日治疗1次,共21 d。大鼠于第22天行超声心动图机检测大鼠左室舒张末期内径(LVEDd)、左室收缩末期内径(LVESD)、左室射血分数(EF)和左室短轴缩短率(FS);TUNEL法检测大鼠心肌细胞凋亡指数(AI)。结果模型组大鼠LVEDd、LVESD高于正常组(P<0.01),而LVEF、LVFS值均低于正常组(P<0.01);内关组和标配组LVEDd、LVESD均低于模型组(P<0.01),标配组低于内关组(P<0.01,P<0.05);内关组和标配组中LVEF、LVFS值均高于模型组(P<0.01),标配组高于内关组(P<0.05)。正常组大鼠心肌AI值低于模型组(P<0.01);内关组、标配组均低于模型组(P<0.01);标配组低于内关组(P<0.05)。结论电针能够通过降低心肌缺血模型大鼠的LVEDd、LVESD值,提升LVEF、LVFS值,有效改善心肌缺血细胞凋亡导致的代偿性心脏扩张、心功能低下,降低心肌细胞凋亡程度,且"标本配穴"电针法比单纯内关电针法具有更好的效应。Objective To investigate the efficacy of“secondary-primary point combination”electroacupuncture by observing its effect on cardiac function and apoptosis index in myocardial ischemia rat to explore the mechanism by which acupuncture pretreatment protects cardiac muscles and provide a scientific method for clinical treatment of this disease.Method Forty rats were allocated,using a random number table,to normal,model,Neiguan and secondary-primary point combination groups.The model was made by 14 consecutive days of abdominal subcutaneous injection of isoprenaline.The normal and model groups were only grabbed and fastened and not given acupuncture.The Neiguan group received acupuncture at bilateral points Neiguan(PC6)and the secondary-primary point combination group,at point Guanyuan(CV4)and bilateral points Neiguan and Zusanli(ST36).Acupuncture was given once daily,for a total of 21 days.Left ventricular end-diastolic diameter(LVEDd),Left ventricular end-systolic diameter(LVESD),left ventricular ejection fraction(EF)and left ventricular fractional shortening(FS)were measured by echocardiography in the rats at day 22.Rat myocardial apoptosis index(AI)was determined by TUNEL.Result Cardiac LVEDd and LVESD were larger and EF and FS were lower in the model group than in the normal group(all P<0.01).LVEDd and LVESD were smaller in the Neiguan and secondary-primary point combination groups than in the model group(P<0.01)and smaller in the secondary-primary point combination group than in the Neiguan group(P<0.01,P<0.05),EF and FS were higher in the Neiguan and secondary-primary point combination groups than in the model group(P<0.01)and higher in the secondary-primary point combination group than in the Neiguan group(P<0.05).Rat myocardial AI was lower in the normal group than in the model group(P<0.01),lower in the Neiguan and secondary-primary point combination groups than in the model group(P<0.01)and lower in the secondary-primary point combination group than in the Neiguan group(P<0.05).Conclusion Electroacupunc
