摘 要:目的探讨右美托咪定在神经外科手术患者中的麻醉效果及对认知功能的影响。方法选取2015年12月—2017年1月天津市环湖医院收治的神经外科手术患者112例,随机分为两组,各56例。对照组应用瑞芬太尼联合丙泊酚静脉注射诱导麻醉干预,研究组在对照组麻醉诱导基础上联合右美托咪定进行干预。对比两组患者麻醉效果、炎性因子水平及简易智力状态检查量表(MMSE)评分情况。结果两组神经外科手术患者麻醉效果比较,通过干预研究组患者麻醉效果各项指标评分明显优于对照组(P<0.05);两组患者干预前后炎性因子水平对比方面,研究组与对照组在术前炎性因子各指标水平无明显差别,研究组术后72 h炎性因子水平明显优于对照组(P<0.05);两组神经外科手术患者干预前后MMSE评分方面,术前两组患者评分差异无统计学意义(P>0.05),干预后24 h显著降低,研究组明显优于对照组(P<0.05),72 h后两组患者MMSE评分明显升高,且研究组评分明显高于对照组(P<0.05)。结论神经外科手术患者治疗过程当中,常规麻醉诱导联合右美拖咪定安全可靠,效果较为理想,可有效减少患者麻醉后认知功能障碍的发生几率,苏醒时间短、无明显副作用,临床上应当进一步推广应用。Objective To investigate the effects of dexmetomidine on anesthetic and cognitive function in patients with neurosurgery.Methods 112 surgery cases of Tianjin Huanhu Hospital from December 2015 to January 2015 were randomly divided into two groups,56 cases in each group.Control group was given fentanyl combined propofol intravenous anesthesia induction intervention.Research group received dexmedetomidine more.The anesthetic effect,the level of validation factor and the MMSE scores were compared between the two groups.Results The anesthetic effect in research group was better than that in control group(P<0.05).Before intervention,each index of preoperative level of inflammatory factors has no obvious difference in two groups.Inflammatory factor levels after 72 h in research group were significantly better than those in control group(P<0.05).The surgery of MMSE scores before intervention in two groups had no obvious difference(P>0.05).Scores decreased significantly 24 h after intervention.Scores in research group were better than those in control group(P<0.05).MMSE scores increased significantly after 72 h.MMSE scores in research group were significantly higher than those in control group(P<0.05).Conclusion In the treatment process of neurosurgery,general anesthesia induction plus dexmedetomidine was safe and reliable and has relatively ideal effect.It can effectively reduce the patients'risk of cognitive dysfunction after anesthesia,with short wake up time and less obvious side effects.It is worthy of clinical further popularization and application.
