低磷血症对慢性阻塞性肺疾病急性加重期患者预后的影响

摘　　要：目的探讨低磷血症在慢性阻塞性肺病急性加重期(AECOPD)患者中的发生率及预后的影响。方法选取2012年1月—2016年6月入住重症医学科(ICU)AECOPD患者86例,根据患者血磷检测值分为正常血磷组和低血磷组,比较两组患者Apache Ⅱ评分、肌酐、血钾、血钠、pH、血氧分压(PaO_2)、动脉血二氧化碳分压(PaCO_2)、机械通气时间、ICU入住时间、住院时间、6个月再入院率、住院期间病死率和死亡率。结果低磷血症总体发生率为53.49%,低磷血症组患者平均住院日、ICU入住时间、机械通气时间较正常组均显著延长[(16.38±9.26)vs(4.65±2.20);(8.05±6.91)vs(2.78±1.44);P<0.05)],且6个月生存率明显较正常血磷组降低(P<0.05)。结论低磷血症在AECOPD患者的发病率高,机械通气时间及住院时间延长,提示患者预后不良。Objective To investigate the incidence of hypophosphatemia and its effects on prognosis in patients with acute exacerbatedchronic obstructive pulmonary disease(AECOPD).Methods A totle of 86 patients with AECOPD were enrolled in intensive care unit(ICU)from January 2012 to June 2016.They were divided into normal phosphorus level group(NG)and low phosphorus level group(LG)according to the phosphorus level.The characteristics were compared between two groups,including Apache II score creatinine,blood potassium,sodium,PH,PaO2,PaCO2,mechanical ventilation time,duration of ICU stay,duration of hospitalization,6 month readmission rate,hospital mortality and all-cause mortality.Results The incidence of hypophosphatemia was 53.49%.The average hospital days,ICU stay,mechanical ventilation time in low phosphorus level group were significantly longer than those in the normal group[(16.38±9.26)vs(4.65±2.20);(8.05±6.91)vs(2.78±1.44),P<0.05)].Six months survival rate in low phosphorus level group was significantly less than that in normal phosphorus level group(P<0.05).Conclusions Hypophosphatemia has high incidence in AECOPD patients.Patients has mechanical ventilation time and prolonged hospitalization,suggesting poor prognosis.
