连续性血液净化联合乌司他丁治疗急性呼吸窘迫综合征的临床疗效研究

摘　　要：目的探讨连续性血液净化联合乌司他丁治疗急性呼吸窘迫综合征(ARDS)的临床疗效。方法选择2013年8月—2016年11月于天津市人民医院就诊并接受治疗的ARDS患者68例,根据数字抽签法将患者分为传统组和创新组,每组34例。传统组采用连续性血液净化治疗,创新组在传统组基础上联合乌司他丁治疗。观察两组患者治疗前后血气分析指标[pH值、血氧分压(PaO_2)、动脉二氧化碳分压(PaCO_2)、PaO_2/吸入气氧浓度(FiO_2)]及治疗后临床相关指标[急性生理学及慢性健康状况评分系统(APACHEⅡ评分)、ICU疗养耗时、机械通气耗时)]并进行比较,统计两组患者死亡情况。结果治疗后创新组患者pH值、PaO_2、PaCO_2、PaO_2/FiO_2高于传统组,APACHEⅡ评分低于传统组,ICU疗养耗时、机械通气耗时短于创新组(P<0.05);传统组病死率高于创新组(P<0.05)。结论连续性血液净化联合乌司他丁治疗ARDS的临床疗效确切。。Objective To investigate the clinical effect of continuous blood purification(CBP)combined with ulinastatin(UTI)in the treatment of acute respiratory distress syndrome(ARDS).Methods A total of 68 ARDS patients in the People's Hospital of Tianjin from August 2013 to November 2016 were selected,which were divide into traditional group and innovative group according to digital lottery.The traditional group was treated with CBP,while the innovative group was treated with CBP combined with UTI.The blood gas analysis index[pH value,oxygen partial pressure(PaO2),arterial partial pressure of carbon dioxide(PaCO2),PaO2/FiO2]were observed before and after treatment,after the treatment clinically relevant indicators[acute physiology and chronic health evaluation(APACHEⅡscore),ICU recuperation time,mechanical ventilation time]were compared,death of patients of two groups were statisticsed.Results After treatment,pH value,PaO2,PaCO2,PaO2/FiO2 of the innovation group were higher than the traditional group,APACHEⅡscore was lower than traditional group,ICU recuperation time,mechanical ventilation time were shorter than traditional group(P<0.05).The mortality rate of the innovation group was lower than the traditional group(P<0.05).Conclusion The clinical effect of CBP combined with UTI in the treatment of ARDS is remarkable.
