摘 要:目的利用多b值IVIM双指数模型,初探不同生理条件下腰椎骨髓IVIM各参数值的变化规律以及双指数模型的slow ADC值与单指数模型的standard ADC值之间的差异。方法 116例(男性54例,女性62例)磁共振检查未发现腰椎骨质异常受试者,在3.0T MR扫描仪上接受L3椎体轴位IVIM DWI成像,b值取0,25,50,80,100,150,200,300,500,1000s/mm2。测量椎体骨髓IVIM各参数值。采用SPSS V19.0软件进行统计分析1)57例受试者slow ADC值和standard ADC值进行比较,并对两者进行相关性分析;2)应用多变量方差分析、独立样本T检验或非参数检验探讨性别、年龄对椎体骨髓IVIM各参数值的影响,并对有差异的参数值与年龄进行Pearson相关性分析。P<0.05表示差异有统计学意义。结果 1)slow ADC值与standard ADC值呈正相关(r=0.674,P<0.001),且前者较低(t=-8.34,P<0.01);2)椎体骨髓的slow ADC值与年龄呈负相关,特别是女性呈显著负相关(r=-0.623,P<0.001)。男性椎体的slow ADC值显著低于女性。以50岁为界,女性椎体骨髓的slow ADC有显著差异(P<0.001)。男性各年龄组之间slow ADC值的差异无统计学意义(P>0.05)。结论 slow ADC值与standard ADC值呈正相关,且前者较低。运用IVIM IDW序列测定椎体各参数值可以无创性的了解椎体骨髓的生理状态及变化规律,进而为研究椎体病变的病理生理变化提供依据。Objective To prospectively demonstrate the change rule of all parameters of IVIM in physiological conditions with biexponential analysis of multiple b-value diffusion-weighted imaging(DWI)and to compare the difference between slow ADC obtained with biexponential analysis and standard ADC obtained with monoexponential analysis.Methods Totally,116 patients(54 males and 62 females)without MRI abnormity were enrolled in the study.Axial IVIM diffusion-weighted imaging of Lumbar 3 was obtained by 3.0 T MRI scanner with weighting factor b of 0,25,50,80,100,150,200,300,500 and 1 000 s/mm 2.The IVIM parameters were obtained with monoexponential analysis.SPSS Statistics V19.0 was used for the statistics analysis.The slow ADC and standard ADC of 57 patients were compared for difference and the correlation between the two parameters were evaluated.The multivariate analysis of variance(MANOVA)was used to study the difference of IVIM parameters of vertebral bone marrow in different age and gender.ANOVA or Kruskal-Wallis H test was used for comparison of IVIM parameters of different age groups.A p-value<0.05 was set as statistical significance.Results 1)The slow ADC value was lower than standard ADC value(t=-8.34,P<0.01).And the correlation between the two parameters was positive(r=0.674,P<0.001);2)The significant negative correlation was found in slow ADC value of vertebral bone marrow to age(r=-0.623,P<0.001),especially in female.The slow ADC value of male vertebral bone marrow was significant lower than female.Slow ADC value below the age fifty years was significant higher than that of above fifty years in female(P<0.001).No significant differenes were found between age groups in male(P>0.05).Conclusion The slow ADC value was lower than standard ADC value.And the correlation between the two parameters was positive.All parameters of vertebral bone marrow derived from IVIM can help understanding the physiological process and change rule of bone marrow noninvasively,and helpful to provide the reference for the pathophysiologica
