泰安市综合医院肿瘤医护人员临终关怀知信行现状调查及影响因素研究

摘　　要：目的了解泰安市综合医院肿瘤专业医护人员临终关怀的态度、知识、行为现状及其影响因素,为今后开展临终关怀工作提供理论依据。方法采用自行设计的《医务人员临终关怀调查表》,对泰安市16家综合医院肿瘤专业的医护人员进行随机整群调查,共得到541份有效问卷。结果 541名医护人员临终关怀总均分为3.97,临终关怀态度得分为3.96,临终关怀知识得分为3.82,临终关怀行为得分为3.94,均超过及格水平接近良好;临终关怀评价得分为4.20,达到良好水平。性别、职业、年龄、学历、行政职务、医院级别、家庭形态不同的医护人员,临终关怀得分有差异(P﹤0.05);进行多元线性回归分析,进入逐步回归模型的是医院级别和学历。结论泰安市综合医院肿瘤专业医护人员的临终关怀总体水平处于中等偏上;女性的临终关怀得分高于男性,护士的临终关怀得分高于医生;一级医院医护人员临终关怀知识得分最低;年轻医护人员临终关怀态度得分较低;专科以下学历人员的临终关怀得分最低;护士长的临终关怀态度最好;小家庭型态的医护人员临终关怀评价得分最高。医院级别和学历是临终关怀的影响因素。各级政府及医疗机构应重视不同人口学特征医护人员临终关怀的差异及其影响因素,对医护人员给予多种途径的临终关怀知识培训,并对医护人员的临终关怀服务行为进行督导。Objective:To understand the attitude,knowledge and behavior and influencing factors of hospice care of Oncology professional medical staff in the general hospitals in Taian area,and provide theoretical basis for hospice care in general hospitals.Methods:A randomized questionnaire survey was conducted among the Oncology professional medical staff in 16 general hospitals in Taian area by using the self-designed"Medical staff hospice questionnaire".A total of 541 valid questionnaires were obtained.SPSS22.0 was used for statistical analysis.Results:The total average score of hospice is 3.97,the average score of hospice attitude is 3.96,the average score of hospice knowledge is 3.82 and the average score of hospice behavior is 3.94.They are all for passing level and close to good level.The average score of the hospice evaluation is 4.20,reach good level.There are significant differences in the scores of hospice care among medical staff with different gender,occupation,age,education,administrative duty,levels of hospitals,and family form(P<0.05).Multivariate linear regression analysis showed that the stepwise regression model was hospital level and educational background.Conclusions:The overall level of hospice care of Oncology medical staff is at a moderate level in general hospitals in Taian area.The scores of hospice care of female are higher than that of men,and of nurses are higher than that of doctors.The lowest score of hospice knowledge was in the first level hospitals.Young nurses have lower scores of hospice attitude.The medical staff with junior college education have the lowest score of hospice.The head nurses have the best attitude towards hospice care.The highest score of hospice evaluation was in the small family.The different levels of hospitals and educational background are the influencing factors of hospice care.Governments and Medical institutions should pay attention to the difference of hospice care among medical staff with different demographic characteristics and its influencing factors,training
