小剂量右美托咪定在腰硬联合麻醉老年患者髋关节置换术中的应用

摘　　要：目的探讨小剂量右美托咪定在腰硬联合麻醉老年患者髋关节置换术中的应用效果。方法 200例进行髋关节置换术的老年患者,按照随机数表法分为研究组与对照组,每组100例。研究组患者进行腰硬联合麻醉和小剂量右美托咪定静脉注射麻醉,对照组患者进行腰硬联合麻醉。记录、对比两组患者不同时间段的平均动脉压(MAP)、心率(HR)、血氧饱和度(Sp O2)及两组患者的术中镇静评分、不良反应发生情况。结果两组患者手术前MAP、HR、Sp O2比较差异均无统计学意义(P>0.05);研究组患者给药后、摆放腰硬联合麻醉体位时、手术侧卧时的MAP、HR、Sp O2与对照组比较,差异均具有统计学意义(P<0.05)。研究组患者术中镇静评分为(1.1±0.4)分、不良反应发生率为2%,均低于对照组的(2.5±0.5)分、9%,差异均具有统计学意义(P<0.05)。结论小剂量右美托咪定在腰硬联合麻醉老年患者髋关节置换术中的麻醉效果明确,值得推广使用。Objective To discuss the application effect of low-dose dexmedetomidine in hip replacement of elderly patients with combined spinal and epidural anesthesia.Methods A total of 200 elderly patients with hip replacement were divided by random number table method into research group and control group,with 100 cases in each group.The research group received combined spinal and epidural anesthesia and low-dose dexmedetomidine intravenous anesthesia,and the control group received combined spinal and epidural anesthesia.Record and comparison were made on mean arterial pressure(MAP),heart rate(HR),blood oxygen saturation(SpO2)in different time periods and intraoperative sedation scores and occurrence of adverse reactions between the two groups.Results Both groups had no statistically significant difference in MAP,HR and SpO2 before operation(P>0.05).The research group had statistically significant difference in MAP,HR and SpO2 after medication,put the position of combined spinal and epidural anesthesia and lateral recumbent(P<0.05).The research group had lower intraoperative sedation score as(1.1±0.4)points and incidence of adverse reactions as 2%than(2.5±0.5)points and 9%in the control group,and their difference was statistically significant(P<0.05).Conclusion Low-dose dexmedetomidine shows affirmative anesthesia effect in hip replacement of elderly patients with combined spinal and epidural anesthesia,and it is worthy of promotion and application.
