摘 要:目的探究颅骨缺损修补术后肽网外露应用自体头皮瓣联合自体肋骨移植修复的效果。方法选取2012年10月-2016年10月我院收治的9例颅骨缺损修补术后肽网外露患者为研究对象。采用自体头皮瓣联合自体肋骨移植对患者肽网外露进行修复,记录手术时间、术中出血量及输血情况,观察手术治疗效果。随访9~12个月,观察患者头部形态、肽网外露再次出现情况以及自体移植肋骨成活情况。结果 9例患者平均手术时间(37.91±7.12)min,术中平均出血量(35.54±6.35)ml,均未输血;术后所有患者皮瓣及植皮区全部成活,切口愈合良好,未出现感染,毛发生长良好;随访9~12个月,患者头部形态均显示正常,未出现肽网再次外露现象,自体移植肋骨成活良好,有良好的支撑作用。结论应用自体头皮瓣联合自体肋骨移植对颅骨缺损修补术后肽网外露的创面有良好的修复效果,且手术时间短、出血量少,术后感染几率小,手术安全性较高。Objective To explore the effects of autologous scalp flap combined with autologous rib grafting on the exposure of peptide mesh after repair of skull defects.Methods 9 cases of patients with exposure of peptide mesh after repair of skull defects in our hospital from October 2012 to October 2016 were selected for the study.Autologous scalp flap and autologous rib grafting were used to repair the exposure of peptide mesh,and the operative time,intraoperative blood loss and blood transfusion were recorded,and the effects of surgical treatment were observed.After 9 to 12 months of follow-up,the head morphology,re-occurrence of exposure of peptide mesh and survival of autograft ribs were observed among patients.Results The mean operative time was(37.91±7.12)min and the mean intraoperative blood loss was(35.54±6.35)ml,and there was no blood transfusion among 9 patients.All flaps and skin grafts survived after operation with good wound healing,no infections and good hair growth.After 9 to 12 months of follow-up,the head morphology showed normal,and there was no re-occurrence of exposure of peptide mesh,and autograft ribs survived well and had a good supporting effect.Conclusions Autologous scalp flap combined with autologous rib grafting has a good repair effect on the exposure of peptide mesh after repair of skull defects,and it has short operative time,small blood loss,small postoperative infection rate and high surgical safety.
