摘 要:目的探讨人工肝血浆置换术对重型乙肝患者乙肝病毒脱氧核糖核酸(HBV-DNA)水平、炎症因子等的影响。方法我院接受人工肝血浆置换术治疗50例重型乙肝患者(观察组)和同期接受常规基础药物治疗50例重型乙肝患者(对照组),观察治疗前后两组炎性因子、凝血功能指标、HBV-DNA转阴率、肝功能指标及不良反应发生情况。结果治疗后两组IL-4、TNF-α、ALT、AST、TBi L明显降低,PT、APTT明显缩短,FIB明显升高,且观察组各项指标变化较对照组明显,差异有统计学意义(P<0.05);观察组HBV-DNA转阴率明显高于对照组(P<0.05),但两组不良反应发生率差异无统计学意义(P>0.05)。结论人工肝血浆置换术可明显提高重型乙肝患者肝功能,作用机制可能与其可有效抑制炎性因子、调节凝血功能、降低HBV-DNA水平有关。To investigate the effect of artificial liver plasma replacement on levels of hepatitis B virus DNA(HBV-DNA)and inflammatory factors in patients with severe hepatitis B.Fifty patients with severe hepatitis B who underwent artificial liver plasma replacement in our hospital were taken as observation group and 50 patients with severe hepatitis B who underwent routine drug treatment in our hospital during the same period were designed as control group.The inflammatory factors,coagulation indexes,negative conversion rate of HBV-DNA,liver function indexes and the incidence of adverse reactions were compared between the two groups.After treatment,IL-4,TNF-α,ALT,AST and TBiL were significantly decreased,PT and APTT were significantly shortened,and FIB was significantly increased when compared to before treatment.Moreover,the changes of above mentioned indexes in the observation group were more obvious than those in the control group(P<0.05).After treatment,the negative conversion rate of HBV-DNA in the observation group was significantly higher than that in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Artificial liver plasma replacement can significantly improve the liver function in patients with severe hepatitis B,and the mechanism may be related to the effective inhibition of inflammatory factors,the regulation of coagulation function and the decrease of HBV-DNA level.
