糠酸莫米松乳膏治疗小儿尿道下裂术后尿道狭窄的临床疗效观察

摘　　要：目的观察糠酸莫米松乳膏用于小儿尿道下裂术后尿道狭窄的临床疗效及其安全性。方法将入选的96例小儿尿道下裂患者根据个体病情选择适当的尿道成形术,术后将患儿随机分为两组各48例,试验组患儿于术后4周起应用糠酸莫米松乳膏涂抹手术切口及水肿的包皮组织;对照组术后保持切口清洁干燥,不做特殊处理。观察两组尿道下裂术后尿道狭窄的发生率及尿道狭窄出现后需要手术的比例。结果两组术后1个月最大尿流率比较差异无统计学意义,术后2个月和3个月最大尿流率比较差异均有统计学意义(P<0.05)。试验组术后尿道狭窄的发生率远低于对照组术后尿道狭窄的发生率(P<0.01),且试验组出现尿道狭窄的病例无1例需要手术干预,而对照组有2例尿道狭窄在尿道扩张无效后行尿道狭窄处尿道造瘘(P<0.01)。试验组患者外用糠酸莫米松乳膏后未出现严重不良反应。结论糠酸莫米松乳膏能够有效的预防和治疗尿道下裂术后尿道狭窄,术后尿道狭窄的并发症、不良反应发生率低,符合临床应用的有效性和安全性。To observe the clinical efficacy and evaluate the safety of mometasone furoate cream for the urethral stricture after hypospadias repair.We choose appropriate urethroplasty for selected 96 hypospadias children according to individual condition.After operation,the children were randomly divided into experimental and control groups,48 in each group.The experimental group was applied mometasone furoate cream to daub the incision and the foreskin tissue edema after 4 weeks of operation while the control group was kept the incision clean and dry after operation without special treatment.The incidence of urethral stricture in the two groups was observed.There was no statistically significant difference in the maximum urinary flow rate between the two groups after 1 month of operation.However,there was significant difference in the maximum urinary flow rate after 2 months and 3 months of surgery(P<0.05).The incidence of postoperative stricture formation in the experimental group was much lower than that in the control group(P<0.01).There was no case of urethral stricture in the experimental group needing surgical intervention while the control group had 2 cases of urethral stricture needed the urethra colostomy after invalid urethra expansion(P<0.01).In the experimental group,there was no serious adverse reaction after the treatment of the cream.The mometasone furoate cream can effectively prevent and treat the postoperative urethral stricture of urethroplasty and postoperative complications of urethral stricture.It has low incidence of adverse reactions in line with the clinical efficacy and safety.
