不同剂量的右美托咪啶用于老年患者臂丛麻醉镇静的临床观察

摘　　要：目的:评价不同剂量的右美托咪啶用于老年患者臂丛麻醉辅助镇静的效果。方法:90例在臂丛麻醉下行上肢手术的老年患者被随机分为D1、D2、D3三组。麻醉效果确切后,D1、D2、D3组分别在10 min内给予不同诱导剂量的右美托咪定:0.2μg/kg、0.4μg/kg、0.6μg/kg,观察T0(给药前)、T1(给药结束)、T2(给药后10 min)、T3(20 min)、T4(30 min)各组患者的Ramsay评分、HR、MAP、RR及Sp O2。结果:D2、D3组T1~T4Ramsay评分高于T0;D2、D3组T1~T4Ramsay评分高于D1组;D3组T1~T4Ramsay评分高于D2组;D2、D3组T1~T4HR低于T0;D2、D3组T1~T4HR低于D1组;D3组T1~T4HR低于D2组。结论:0.4μg/(kg·min)右美托咪啶用于老年患者臂丛麻醉辅助镇静适当。ObjectiveTo evaluate the effect of different doses dexmedetomidine for sedation of elderly patients in brachial plexus anesthesia.Method 90 elderly patients scheduled for upper limb surgeryunder brachial plexus anesthesia were randomly divided into three groups:D 1,D2,D 3.After the anesthesia effect,Patients in D 1,D 2 and D 3 were givendifferent induction doses of dexmedetomidine:0.2μg/kg,0.4μg/kg,0.6μg/kg.TheRamsay scores,HR,MAP,RR and SpO 2 at T 0(before administration),T 1(endof dosing),T 2(10 min after administration),T 3(20 min),T 4(30 min)were observed and recorded.Results Ramsay scores at T 1~T 4 were higher than that of T 0 in group D 2 andD 3(P<0.05),Ramsay scores at T 1~T 4 in group D 2 and D 3 were higher thanthat ingroup D 1(P<0.05)and Ramsay scores at T 1~T 4 in group D 3 werehigher than that in group D 2(P<0.05).HR at T 1~T 4 were lower than that ofT 0 ingroup D 2 and D 3(P<0.05),HR at T 1~T 4 in group D 2 and D 3 werelower thanthat in group D 1(P<0.05)and HR at T 1~T 4 in group D 3 were lowerthan thatin group D2(P<0.05).Conclusion〖STBZ〗0.4μg/(kg·min)dexmedetomidine is appropriate for sedation of elderlypatients undergoing brachial plexus anesthesia.
