依托咪酯在老年患者眼科日间手术中的应用

摘　　要：目的:观察依托咪酯用于合并心脑血管疾病老年患者眼科日间手术的麻醉效果。方法:60例ASAⅡ~Ⅲ级行睑内翻矫正术患者随机分为两组,局部麻醉组即利多卡因组(L组)和依托咪酯组(E组),每组30例。分别记录各组患者入室前(T1)、术中10 min(T2)、手术结束时(T3)、术后30 min(T4)4个时间点的收缩压(SBP)、舒张压(DBP)、心率(HR)、脉搏血氧饱和度(Sp O2)。记录各组手术时间、术中术后严重不良反应发生情况和镇静评分等。结果:L组SBP、DBP在T2、T3、T4时比T1升高,差异有统计学意义(P<0.01);HR在T2、T3、T4时比T1升高,差异有统计学意义(P<0.05);E组SBP、DBP在T2、T3、T4时比T1下降,差异有统计学意义(P<0.05);HR在T2、T3、T4时比T1变化不明显,差异无统计学意义(P>0.05);E组患者镇静评分高于L组,差异有统计学意义(P<0.01)。结论:依托咪酯可安全用于合并心脑血管疾病的老年患者眼科日间手术麻醉。Objective Observation of the anesthetic effect of etomidate in the daytime operation of the elderly patients with cardiovascular and cerebrovascular diseases.Method 60 patients receiving correction of entropion of ASAⅡ~Ⅲlevel were divided into two groups.Each group,namely 30 patients,were injected with Lidocaine respectively for local anesthesia(group L)and etomidate respectively for intravenous anesthesia(group E).The systolic blood pressure(SBP),diastolic blood pressure(DBP),heart rate(HR)and pulse Oxygen Saturation(SpO 2)of the patients from two groups were recorded before the surgery(T 1),10 minutes in the duration of the surgery(T 2),at the completion of the surgery(T 3)and 30 minutes after the surgery(T 4)respectively the time of surgery,raking ofsevere untoward effect and Ramsay scores of each group during and after the surgery are traced.Results SBPand DBP of group L were higher at T 2,T 3,T 4 than at T 1(P<0.01).HRof groupLwas also higher at T 2,T 3,T 4 than at T 1(P<0.05).Whereas,SBP and DBPof group E were lower at T 2,T 3,T 4 than at T 1(P<0.01),while there wasno significant change of HR at T 2,T 3,T 4 as compared to at T 1(P>0.05).Sedation of group Eranks higher than group L(P<0.01).Conclusion Etomidate can be safely used as an ophthalmic anesthetic in the daily operation of cardiovascular&cerebrovascular patients.
