摘　　要：目的构建适合三甲医院的延续性护理模式,指导临床护理实践。方法对519例住院患者进行延续性护理需求调查,基于调查结果形成专家咨询问卷,并对23名专家进行2轮Delphi专家咨询,构建三甲医院的延续性护理模式。结果构建含"导向"、"评估时机"、"组织架构"、"内容"、"方式"、"期限"、"效果评价标准"7个一级指标,16个二级标及12个三级指标的三甲医院延续性护理模式。结论基于患者需求及Delphi专家咨询法构建的三甲医院延续性护理模式,具有一定的科学性和实用性。Objective To construct a transitional care model suitable for grade three hospitals,and to guide clinical nursing practice.Methods A total of 519 inpatients were investigated for their needs of transitional care,and an expert consultation questionnaire was formed based on the investigation results,then 2 rounds Delphi expert consultation were conducted on 23 experts to construct a transitional care mode for grade three hospitals.Results The transitional care model for grade three hospitals was constructed,which consisted of 7 first level indicators(guidance,assessment time,organizational structure,content,way,deadline and effect evaluation standard),16 second level indicators and 12 three level indicators.Conclusion The transitional care model for grade three hospitals based on patient needs and Delphi method is scientific and practical.
