改良型中等长度导管与PICC在胃肠道肿瘤患者围手术期中的应用比较

摘　　要：目的改良型中等长度导管(Midline catheters)与经外周静脉置入中心静脉导管(PICC)应用于胃肠道肿瘤患者围手术期治疗过程中导管相关并发症及医疗成本的分析比较。方法选取2016年3月至2017年4月就诊于我院普外科诊断为胃肠道恶性肿瘤患者136例,随机等分为观察组和对照组,观察组在围手术期采用改良型中等长度导管进行相关静脉输液,对照组采用PICC进行相关静脉输液。比较两组患者置管后导管相关并发症的发生情况,同时对两组患者导管相关医疗成本与患者医疗花费进行比较。结果观察组患者导管相关并发症的发生情况明显低于对照组(P<0.05)。观察组置管后导管维护平均费用及导管相关平均总花费均较对照组明显减少(P<0.05)。结论改良型中等长度导管可用于围手术期胃肠道肿瘤患者化疗、静脉营养的治疗中,并且导管相关并发症及医疗成本较PICC的均显著降低。Objective The improved midline catheters and peripherally inserted central venous catheters(PICC)were used to analyze the catheter-related complications and medical costs during perioperative treatment of patients with gastrointestinal tumors.Methods To select 136 patients with gastrointestinal malignant tumor who had been diagnosed in general surgery department in our hospital from March 2016 to April 2017,randomly and equally divided them into observation group and control group,the observation group in the perioperative used modified midline catheters to carry out related intravenous infusion,the control group adopted PICC to carry out relative intravenous infusion.Compare the incidence of catheter related complications in two groups of patients after accepted cathetering,and compare the catheter related medical costs in the two groups with patients'medical expenses.Results The incidence of catheter related complications in the observation group was significantly lower than that in the control group(P<0.05).The average cost of catheter maintenance and catheter related average total cost after accepted cathetering in the observation group was significantly less than that in the control group(P<0.05).Conclusion The improved midline catheters can be used in the chemotherapy and the parenteral nutrition applied to patients with gastrointestinal tumor,and catheter related complications and medical cost were significantly reduced than PICC.
