生长抑素用于老年术后早期炎性肠梗阻的临床研究

摘　　要：目的探讨生长抑素用于老年术后早期炎性肠梗阻(EPISBO)的临床疗效。方法将119例EPISBO老年患者随机分为对照组(59例)和治疗组(60例),两组患者均给予常规治疗,治疗组患者加用生长抑素。连续治疗7 d。结果治疗组的总有效率为93.33%,明显高于对照组的72.88%(χ~2=8.899,P=0.003);治疗组的腹部症状缓解时间、肠鸣音恢复时间、肛门排气恢复时间、恶心消失时间、胃肠减压引流量及治愈时间均明显短于对照组(P<0.05);治疗后,两组患者血清C反应蛋白(CRP)、白细胞介素6(IL-6)及肿瘤坏死因子-α(TNF-α)水平均比治疗前降低,且治疗组比对照组降低更明显(P<0.05)。结论生长抑素辅助治疗老年术后EPISBO临床疗效较好,能迅速改善临床症状,缩短病程,减轻肠道炎性反应,且安全性较好,值得临床推广。Objective To investigate the clinical effect of somatostatin in treating elderly patients with early postoperative inflammatory small bowel obstruction(EPISB0).M ethods Totally 119 elderly patients with EPISB0 were randomly divided into the control group(59 cases)and the treatment group(60 cases).The control group was treated with conventional treatment,while the treatment group were treated with conventional treatment combined with somatostatin.The two groups were treated for 7 d.Results The total effective rate of the treatment group was 93.33%,which was significantly higher than 72.88%of the control group(;^2=8.899,P=0.00 3).The abdominal symptoms relief time,bowel sounds recovery tim e,anus exhaust recovery time,nausea disappear time,gastrointestinal decompression drainage volume and cure time of the treatment group were significantly shorter than those of the control group(<0.0 5).After treatment,the serum levels of CRP,IL-6 and TNF-a in the two groups were significantly lower than those before treatment,and those in the treatment group were significantly lower than those in the control group(<0.05).Conclusion Somatostatin in the treatment of elderly patients with EPISB0 has good effect and safety,it can quickly improve the clinical symptoms,shorten the course of the disease and reduce the intestinal inflammation reaction,which is worthy of clinical promotion.
