利拉鲁肽联合胰岛素治疗2型糖尿病过程中血糖变化及安全性分析

摘　　要：目的通过血糖监测评价利拉鲁肽联合胰岛素治疗2型糖尿病的疗效及安全性。方法选取2016年4月至2017年4月内分泌科住院的2型糖尿病患者,按照标准纳入患者40例,随机分为利拉鲁肽联合胰岛素治疗组和单用胰岛素治疗组。分别对观察者在治疗前、治疗3个月后进行3 d的动态血糖监测,具体监测类型包括:日间血糖差、血糖最高幅度、用餐之后血糖变化量,对患者进行治疗时,定期监测其空腹血糖值、用餐2 h血糖值、糖化血红蛋白。结果治疗3个月后空腹血糖、糖化血红蛋白、餐后2 h血糖2组间差异无统计学意义(P>0.05)。而患者血糖波动、日间血糖差、血糖最高幅度、用餐后患者血糖变化量间差异有统计学意义(P<0.05)。结论利拉鲁肽联合胰岛素可以有效控制2型糖尿病患者血糖,可以减少血糖波动,降低低血糖发生率,使2型糖尿病治疗更具安全性。Objective To evaluate the therapeutic effects and safety of liraglutide combined with insulin in treatment of type 2 diabetes mellitus through blood glucose monitoring.Methods Forty patients with type 2 diabetes mellitus who were treated in our hospital from April 2016 to April 2017 were randomly divided into combination treatment group(liraglutide+insulin)and ximple insulin treatment group.The blood glucose indexes including blood glucose fluctuations,the absolute difference of daytime blood sugar,the highest extent of blood sugar,postprandial blood glucose range,fasting blood glucose,postprandial 2h blood glucose,glycated hemoglobin before treatment,during treatment,at 3 months after treatment were observed and compared between two groups.Results After 3-month treatment,there were no significant differences in the levels of fasting blood glucose,glycated hemoglobin,2h postprandial blood glucose between two groups(P>0.05).However there were significant differences in blood glucose fluctuations,the absolute difference of daytime blood sugar,the highest extent of blood sugar,postprandial blood glucose range between two groups(P<0.05).Conclusion The liraglutide combined with insulin can effectively control blood sugar in patients with type 2 diabetes mellitus,and can reduce blood sugar fluctuations,decrease the incidence of hypoglycemia so as to increase security in treatment of type 2 diabetes mellitus.
