复合麻醉在脊柱侧弯矫形手术中的危险因素分析及临床预后研究

摘　　要：目的探讨复合麻醉在脊柱侧弯矫形手术治疗中的危险因素及临床预后。方法采用复合麻醉在脊柱侧弯矫形手术治疗的60例住院患者临床资料,包括基本资料、病史、生活习惯、实验室指标、手术中出血量的特点等,采用多因素回归分析探讨复合麻醉在脊柱侧弯矫形手术治疗中的危险因素分析及临床预后。结果患者是否存在高血压、平时血压水平、术中出血量、术前血糖水平是复合麻醉下脊柱侧弯矫形手术治疗的危险因素,与预后相关,其中术前的血糖水平与预后差异有统计学意义(P<0.01)。结论复合麻醉下脊柱侧弯矫形手术治疗中患者有无高血压、血压控制情况、术中出血量、术前血糖水平是相关危险因素,与手术预后相关,提示临床手术治疗前应该关注患者的血糖和血压水平,积极将其控制在正常范围。Objective To investigate the risk factors and clinical prognosis of combined anesthesia in the surgical treatment of scoliosis.Methods The clinical data of 60 inpatients with scoliosis who were admitted and received treatment of combined anesthesia in orthomorphia from January 2013 to December 2014 in our hospital were retrospective analyzed.The clinical data included basic information,medical history,living habit,laboratory parameters,characteristics of intraoperative bleeding volume.The risk factors and prognosis of combined anesthesia in surgical treatment of scoliosis were analyzed by means of multivariate regression analysis.Results The multivariate regression analysis results showed that high blood pressure,the usual blood pressure levels,intraoperative bleeding volume,preoperative blood glucose levels were the risk factors,moreover,which were related with clinical prognosis of patients with scoliosis after surgery with complex anesthesia,in which,there were significant differences in preoperative blood glucose levels and prognosis differences(P<0.01).Conclusion In the scoliosis surgery under compound anesthesia,high blood pressure,the condition of blood pressure controlling,intraoperative bleeding volume,preoperative blood glucose levels are the related risk factors with prognosis of patients,which suggests that the levels of blood glucose and blood pressure levels should be paid much attention before surgery,therefor,it is necessary to control them in normal range.
