微波理疗在肿瘤患者PICC穿刺置管中的应用观察

摘　　要：目的探讨微波理疗在肿瘤患者行中心静脉置管PICC穿刺置管中提高穿刺成功率并减少置管不良反应的应用效果。方法采用随机对照原则,将符合纳入排除标准的行PICC导管置入肿瘤患者300例纳入MST组和MST+微波理疗组,每组150例。MST组正常置管,MST+微波理疗组置管前对穿刺血管行微波理疗。比较2组患者一次穿刺成功率、患者疼痛程度和满意度、血管痉挛和静脉炎的发生情况,比较MST+微波理疗组微波理疗前后血管内径大小。结果 MST+微波理疗组微波理疗后血管内径较微波理疗前明显扩张,一次性穿刺成功率明显高于MST组(P<0.05)。MST+微波理疗组的血管痉挛发生率及平均送管时间均明显小于MST组(P<0.05)。结论肿瘤患者行PICC穿刺部位微波理疗有助于扩张患者血管内径,减轻患者疼痛,提高一次穿刺成功率,减少血管痉挛和静脉炎的发生,从而减少反复穿刺给患者带来的痛苦,大大提高患者满意度,临床效果很好,值得推广。Objective To explore the application effects of microwave physiotherapy in improving the success rate of puncture and reducing the adverse reaction of catheterization in patients with neoplasms.Methods According to random control principle,300 patients with tumor who were ready to receive PICC catheterization were divided into MST group(n=150)and MST+microwave therapy group(n=150).The patients in MST group were treated by normal catheterization,however,the patients in MST+microwave therapy group were treated by puncture vascular microwave physiotherapy before catheterization.The one time success rate of puncture,the pain extent and satisfaction degree,the incidence rates of vasospasm and phlebitis were observed and compared between two groups.Moreover the blood vessels internal diameter size in MST+microwave therapy group was compared before and after microwave therapy.Results After treatment the blood vessels internal diameter in MST+microwave therapy group was significantly greater than that before treatment,moreover,one time success rate of puncture in MST+microwave therapy group was significantly higher than that in MST group(P<0.05).In addition the incidence rate of vasospasm and average delivering catheter time in MST+microwave therapy group were significantly lower than those in MST group(P<0.05).Conclusion The puncture vascular microwave physiotherapy is beneficial to expand blood vessel internal diameter,relieve pain of patients with tumor,improve one time puncture success rate,reduce the incidence of vasospasm and phlebitis,so as to improve clinical effects and satisfaction degree of patients,therefor,which is worth using widely in clinical practice.
