自体角膜缘干细胞移植联合翼状胬肉切除术临床效果评价

摘　　要：目的探讨自体角膜缘干细胞移植联合翼状胬肉切除术的临床价值。方法 2014年1月—2016年10月该院翼状胬肉患者100例随机分为观察组及对照组各50例,对照组进行单纯胬肉切除手术,观察组在翼状胬肉切除术的基础上实施角膜缘干细胞移植,观察指标包括:角膜上皮修复时间、住院时间、术后并发症、痊愈标准、复发发生率。结果观察组患者角膜上皮修复时间平均为(3.79±1.04)d、住院时间为(7.20±1.32)d,短于对照组的(5.65±1.27)、(9.50±1.44),(t=3.45、2.87,P<0.05);治疗效果痊愈98.00%高于对照组的84.00%(χ~2=4.98,P<0.05);两组术后并发症(2.00%vs 4.00%),差异无统计学意义(χ~2=0.23,P>0.05)。对两组患者进行随访1~3年,平均(1.39±0.55)年,观察组复发6.00%低于对照组的28.00%(χ~2=8.23,P<0.05)。结论翼状胬肉切除术联合自体角膜缘干细胞移植术疗效显著,创面修复时间短,复发率低,安全性高,可作为基层医院临床首选方法。Objective To study the clinical value of autologous limbal stem cell transplantation combined with pterygium resection.Methods 100 cases of patients with pterygium admitted and treated in our hospital from January 2014 to October 2016 were selected and randomly divided into two groups with 50 cases in each,the control group used the simple pterygium resection,while the observation group used the autologous limbal stem cell transplantation combined with pterygium resection,and the epithelium corneae repair time,length of stay,postoperative complications,healing standards and incidence rate of recurrence were observed.Results The average repair time of epithelium corneae and length of stay in the observation group were shorter than those in the control group[(3.79±1.04),(7.20±1.32)d vs(5.65±1.27),(9.50±1.44)d],(t=3.45,2.87,P<0.05),and the healing effect in the observation group was higher than that in the control group,(98.00%vs 84.00%),(χ2=4.98,P<0.05);the difference in the postoperative complications between the two groups was not obvious,(2.00%vs 4.00%),(χ2=0.23,P>0.05),and the average follow-up time of the two groups was(1.39±0.55)years,(1~3)years,and the recurrence was lower than that in the control group,(6.00%vs 28.00%)(χ2=8.23,P<0.05).Conclusion The curative effect of autologous limbal stem cell?transplantation combined with pterygium resection is obvious,with short wound repair time,low recurrence rate and high safety,which can be firstly used in the primary hospitals.
