肿瘤恶性积液顺铂热灌注治疗疗效分析

摘　　要：目的综合分析肿瘤恶性积液顺铂热灌注治疗效果。方法选取在该院(在2013年4月—2017年8月)收治的80例肿瘤恶性胸、腹水患者,按照入院顺序随机分为实验组(应用顺铂热灌注联合复方苦参注射液静脉给药治疗方法,40例)与对照组(引流胸腹水后应用复方苦参注射液静脉给药治疗方法,40例)。采用SPSS 20.0统计学软件进行统计学分析两组患者的近期临床治疗总有效率、不良反应发生率以及治疗前后体力状况评分等参数指标。结果实验组近期临床治疗总有效率为87.50%(35/40,10例完全缓解、22例部分缓解、3例稳定、5例进展,),对照组近期临床治疗总有效率为72.50%(29/40,5例完全缓解、8例部分缓解、16例稳定、11例进展);实验组患者的体力状况评分为(62.22±4.36)分,对照组为(62.25±4.31)分,两组患者的体力状况评分比较差异无统计学意义(P>0.05);治疗后,实验组患者体力状况评分为(89.66±1.15)分,对照组为(75.55±1.96)分,实验组患者的体力状况评分显著优于对照组患者(P<0.05),两组患者在发热、肝功能损害和肠梗阻等不良反应发生率比较差异无统计学意义(P>0.05),实验组患者在恶心呕吐、腹痛以及腹壁渗液等不良反应发生率显著低于对照组患者(P<0.05)。结论肿瘤恶性积液顺铂热灌注联合复方苦参注射液静脉给药治疗效果显著高于单独复方苦参注射液静脉给药治疗方法。Objective To comprehensively analyze the curative effect of intrapleural hyperthermic perfusion of cisplatin in treatment of tumor malignant effusion.Methods 80 cases of patients with tumor malignant hydrothorax or hydroperitoneum admitted and treated in our hospital from April 2013 to August 2017 were selected and randomly divided into two groups with 40 cases in each,the experimental group used the intrapleural hyperthermic perfusion of cisplatin combined with intravenous administration of kushen injection,while the control group used the intravenous injection of kushen after the drainage of hydrothorax or hydroperitoneum,and the total clinical treatment effective rate,incidence rate of adverse reactions and physical state score before treatment were statistically analyzed.Results The total treatment effective rate in the experimental group and in the control group was respectively 87.50%and 72.50%,respectively(35/40,10 cases with complete remission,22 cases with partial remission,3 steady cases,5 progressed cases)and(29/40,5 cases with complete remission,8 cases with partial remission,16 steady cases,11 progressed cases),and the physical state score in the experimental group and in the control group was respectively(62.22±4.36)points and(62.25±4.31)points,and the difference was statistically significant(P>0.05),after treatment,the physical state score in the experimental group was obviously better than that in the control group,[(89.66±1.15)points vs(75.55±1.96)points](P<0.05),and the difference in the incidence rate of adverse reactions of fever,liver function injury and intestinal obstruction between the two groups was not statistically significant(P>0.05),and the incidence rate of adverse reactions of nausea and vomiting,abdominal pain and abdominal wall solution in the experimental group was obviously lower than that in the control group(P<0.05).Conclusion The effect of intrapleural hyperthermic perfusion of cisplatin in treatment of tumor malignant effusion is obviously higher than that of intravenou
