硬质三维成形环治疗左心系统瓣膜疾病合并三尖瓣关闭不全的疗效探讨

摘　　要：目的探讨硬质三维成形环治疗左心系统瓣膜疾病合并三尖瓣关闭不全的疗效。方法收集2013年2月—2016年6月在第三军医大学接受硬质三维成形环治疗的102例左心系统瓣膜疾病合并三尖瓣关闭不全患者临床资料,分析其临床疗效及预后。结果102例患者中术后早期死亡3例(2.94%),发生并发症者9例(8.82%);术前经超声心动图检查:7例为三尖瓣轻度反流(6.86%),50例中度反流(49.02%),45例重度反流(44.12%);出院前经检查,95例为三尖瓣反流轻度以下(93.14%),7例为中度反流(6.86%),无重度反流者;术前右室大小为(29.73±7.52)mm,出院前右室大小为(23.21±5.63)mm,差异有统计学意义(P<0.05);20例患者术后1年门诊随访时经检查,18例为三尖瓣反流轻度以下(90.00%),2例(10.00%)为中度反流,无重度反流者。结论对左心系统瓣膜疾病合并三尖瓣关闭不全患者实施硬质三维成形环治疗效果显著。Objective To study the curative effect of three-dimensional annuloplasty ring in treatment of left-heart valvular heart disease combined with tricuspid valve insufficiency.Methods 102 cases of patients with left-heart valvular heart disease combined with tricuspid valve insufficiency treated in Thivd Military Medical University from February 2013 to June 2016 were selected and the clinical curative effect and prognosis were analyzed.Results Of 102 cases of patients,there were 3 cases with postoperative early death(2.94%),9 cases with complications(8.82%),and the ultrasonic cardiogram before surgery showed that there were 7 cases with light tricuspid regurgitation(6.86%),50 cases with moderate regurgitation(49.02%)and 45 cases with severe regurgitation(44.12%),and the examination before discharge showed that there were 95 cases with light tricuspid regurgitation or below(93.14%),7 cases with moderate regurgitation(6.86%)and there were no severe regurgitation,and the sizes of the right ventricle before surgery and before discharge were respectively(29.73±7.52)mm and(23.21±5.63)mm,and the differences were statistically significant(P<0.05),of 20 cases of patients examined after 1 year of outpatient follow-up,there were 18 cases with light tricuspid regurgitation or below(90.00%)and 2 cases(10.00%)with moderate regurgitation and 0 case with severe regurgitation.Conclusion The curative effect of three-dimensional annuloplasty ring in treatment of left-heart valvular heart disease combined with tricuspid valve insufficiency is obvious.
