螺旋CT与MRI用于强直性脊柱炎骶髂关节病变诊断中的对照分析

摘　　要：目的对比螺旋CT检查、MRI检查应用在强直性脊柱炎骶髂关节病变诊断中的价值,总结临床经验。方法选择该院2015年6月—2017年6月收治的53例强直性脊柱炎患者作为研究对象,分别进行螺旋CT检查与MRI检查。对比检查结果。结果 MRI骶髂关节病变Ⅰ级的诊断率为35.85%,高于螺旋CT;骶髂关节突病变、骨髓水肿的检出率MRI检查也更高,分别为77.36%、86.79%;关节面增生硬化、关节面侵蚀的检出率螺旋CT检查更高,分别为90.57%、66.04%,差异有统计学意义(P<0.05)。结论螺旋CT检查、MRI检查诊断强直性脊柱炎骶髂关节病变各有各的优势,可联合使用,提高检查诊断效果。Objective To compare the value of spiral CT and MRI in diagnosis of sacroiliac joint in ankylosing spondylitis and summarize the clinical experience.Methods 53 cases of patients with ankylosing spondylitis admitted and treated in our hospital from June 2015 to June 2017 were selected as the research objects and divided into two groups,respectively examined by the spiral CT and MRI,and the examination results were compared.Results The diagnosis rate of MRI in diagnosis of sacroiliac joint levelⅠwas higher than that of spiral CT,and the test rate of MRI in diagnosis of sacroiliac joint disease and bone marrow edema was higher,respectively 77.36%and 86.79%,and the test rate of spiral CT in diagnosis of hyperplasia hardening of joint surface and erosion of joint surface was higher,respectively 90.57%and 66.04%,the difference was statistically significant(P<0.05).Conclusion Spiral CT examination and MRI examination in diagnosis of sacroiliac joint in ankylosing spondylitis each have its advantages,and the combination of the two can improve the diagnosis effect.
