普罗帕酮联合胺碘酮治疗急性心肌梗死并发心房颤动临床效果对比分析

摘　　要：目的探索普罗帕酮与胺碘酮联合治疗急性心肌梗死并发心房颤动的效果。方法 2016年3月—2017年2月在该院就诊被确诊为急性心肌梗死并发心房颤动患者中选出174例随机分组,其中对照组87例患者给予胺碘酮治疗,观察组87例患者加用普罗帕酮治疗,对比治疗效果指标。结果观察组患者在6 h、6~12 h以及24 h内的转复率分别为37.93%、43.68%、98.85%;对照组分别为24.14%、28.74%、81.61%,观察组均显著高于对照组患者(P<0.05);观察组患者在12~24 h的转复率17.24%与对照组28.74%对比差异无统计学意义(P>0.05);观察组患者的不良反应发生率6.90%与对照组的8.05%比较差异无统计学意义(P>0.05);两组患者不良反应均较为轻微。结论普罗帕酮与胺碘酮联合用于急性心肌梗死并发心房颤动治疗中效果确切,能快速促使心律的转复,且安全性高,值得推广。Objective This paper tries to investigate the efficacy of propafenone combined with amiodarone in the treatment of acute myocardial infarction complicated with atrial fibrillation.Methods 174 cases diagnosed with acute myocardial infarction complicated by atrial fibrillation from March 2016 to February 2017 in this hospital were selected and randomly divided into two groups.87 patients in the control group were treated with amiodarone.87 cases in the observation group received propafenone treatment,and the therapeutic effect index was compared.Results The cardioversion rate of patients in the observation group in 6 h,6~12 h and 24 h were 37.93%,43.68%,98.85%;and that in the control group were 24.14%,28.74%,81.61%,the observation group were significantly higher than that in control group(P<0.05);The patients in the observation group of 12~24 h,the cardioversion rate was 17.24%and that of the control group was 28.74%with no significant difference between the two groups(P>0.05);Observation group with adverse reaction rate of 6.90%and 8.05%in the control group,with no significant difference(P>0.05);The adverse reactions in the two groups were relatively minor.Conclusion Propafenone combined with amiodarone is effective in the treatment of acute myocardial infarction complicated with atrial fibrillation.It can rapidly promote the cardioversion of heart rhythm,and it has high security and is worthy of promotion.
