摘 要:目的探讨三维快速自旋回波(3D sampling perfection with application optimized contrasts using different flip angle evolutions,3D-SPACE)与二维快速自旋回波(2D proton density weighted image,2D PDWI)在诊断膝交叉韧带与半月板损伤中的效果。方法选取2015年8月—2017年5月间于该院检查的发生膝部外伤的患者共106例,对患者分别进行2D PDWI及3D-SPACE扫描,分别对2种检查结果获得的图像进行主观评分,以关节镜作为诊断的金标准进行对比分析。结果将3D-SPACE扫描获得的结果记为实验组,将2D-PDWI扫描获得的结果记为对照组。对照组1分16例、2分6例、3分32例、4分52例;实验组1分28例、2分26例、3分88例、4分176例,得出实验组的主观评分同对照组相比较低,差异有统计学意义(χ2=6.460,P=0.016);实验组患者经关节镜确诊后,出现误诊2例;对照组患者经关节镜确诊后,出现误诊6例,实验组方法同关节镜的吻合程度高于对照组方法(t=17.781,P=0.001)。结论 3D SPACE较2D PDWI技术在诊断膝交叉韧带与半月板损伤中的效果显著,诊断准确性较好,且速度快,值得于临床中推广。Objective This paper tries to explore the effect of 3D sampling perfection with application optimized contrasts using different flip angle evolutions,(3D-SPACE)and 2D proton density weighted image,(2D PDWI)in the diagnosis of knee cruciate ligament and meniscus injury.Methods 106 patients with knee trauma who were examined in this hospital from August 2015 to May 2017 were selected.2D PDWI and 3D-SPACE scans were performed on the patients.Subjective scores were obtained for the two images,regarding the arthroscopy as a diagnostic gold standard for comparative analysis.Results The results obtained by the 3D-SPACE scanning were taken as the experimental group,the results obtained by the 2D-PDWI scanning were taken as the control group.In the experimental group,1 score of 16 cases,2 score of 6 cases,3 score of 32 cases,4 score of 52 cases;in the control group,1 score of 28 cases,2 score of 26 cases,3 score of 88 cases and 4 score of 176 cases.The subjective score of the experimental group was lower than control group,with significant difference(χ2=6.460,P=0.016);The patients in the experimental group were diagnosed by arthroscopy and misdiagnosed in 2 cases.In the control group,6 cases were misdiagnosed after arthroscopy.The anastomosis of the experimental group was higher than that of the control group(t=17.781,P=0.001).Conclusion 3D SPACE is more effective than 2D PDWI in the diagnosis of knee cruciate ligament and meniscus injury.The diagnostic accuracy is better and faster,and it is worthy to be popularized in clinical practice.
