快速康复理念在全髋关节置换术围手术期的临床应用

摘　　要：目的探讨快速康复(ERAS)理念在全髋关节置换术围手术期的临床应用价值。方法以我院2015年6月至2016年12月收治的98例行单侧髋关节置换术的患者为研究对象,将其随机分为对照组(n=48,接受传统模式治疗)与观察组(n=50,接受快速康复治疗)。对比两组手术切口、手术时间、出血量、术后并发症、术后恢复情况、Harris评分及评分优良率及住院时间、住院费用等经济指标。结果观察组患者的切口长度、手术时间、出血量、术后3 d内下床率、7 d内辅助行走率及Harris评分均优于对照组(P<0.05);观察组术后住院天数、住院总天数均短于对照组,术后费用及住院总费用均少于对照组(P<0.05)。结论围手术期应用快速康复理念使全髋关节置换术患者更快康复,费用更低,住院时间更短,值得广泛推广。Objective To explore the clinical application value of enhanced recovery after surgery(ERAS)concept in perioperative period of total hip replacement.Methods From June 2015 to December 2016,98 patients with unilateral hip replacement in our hospital were selected as the subjects and randomly divided into control group(n=48,treated with traditional treatment methods)and observation group(n=50,treated with ERAS treatment methods).The length of the incision,operation time,blood loss,postoperative complications,postoperative recovery,Harris scores and excellent and good rates of scores were compared between the two groups.The economic indicators that hospital stays,hospital expenses were also compared.Results The length of the incision,operation time,blood loss,ambulation rate in 3 d after surgery,aided walking rate in 7 d after surgery and Harris score in the observation group were significantly better than those in the control group(P<0.05).The postoperative days of hospitalization and total days of hospitalization in the observation group were shorter than those in the control group,the postoperative costs and the total hospitalization costs were less than those in the control group(P<0.05).Conclusion Perioperative application of ERAS concept in perioperative period of total hip replacement can make total hip replacement patients recover faster,with less costs and shorter hospital stays,which is worth popularizing widely.
