宫腔镜下子宫内膜电切术与左炔诺孕酮宫内节育系统治疗功能性子宫出血的效果比较

摘　　要：目的比较宫腔镜下子宫内膜电切术与左炔诺孕酮宫内节育系统治疗功能性子宫出血的临床效果。方法将92例功能性子宫出血患者按随机数表法分为对照组和观察组,每组46例。对照组采用左炔诺孕酮宫内节育系统治疗,观察组采用宫腔镜下子宫内膜电切术治疗。观察对照组治疗前、观察组术前及2组随访1个月性激素指标[血清促卵泡生成素(FSH)、促黄体生成素(LH)、垂体催乳素(PRL)、孕酮(P)、睾酮(T)、雌二醇(E2)]水平的变化和并发症(阴道不规则出血、下腹坠胀痛)发生率。结果观察组随访1个月血清FSH、LH、E2、PRL、P、T水平与术前比较差异无统计学意义(P>0.05)。与同组治疗前比较,对照组随访1个月血清FSH、LH、PRL、P、T水平均明显升高,血清E2水平明显降低(均P<0.05);与对照组比较,观察组随访1个月血清FSH、LH、PRL、P、T水平和并发症发生率均明显降低,血清E2水平明显升高(均P<0.05)。结论宫腔镜下子宫内膜电切术治疗功能性子宫出血,对卵巢功能无明显的影响,并发症发生率较低,安全性较高。Objective The compare the clinical effect of hysteroscopic endometrium electrosection and levonorgestrel intrauterine system on functional uterine bleeding.Methods Ninety-two patients with functional uterine bleeding were randomly treated with either levonorgestrel intrauterine system(control group,n=46)or hysteroscopic endometrium electrosection(observation group,n=46).Serum levels of follicle stimulating hormone(FSH),luteinizing hormone(LH),pituitary prolactin(PRL),progesterone(P),testosterone(T)and estradiol(E 2)and incidence of complications(irregular vaginal bleeding and lower abdominal distention and pain)were observed before treatment,after operation and after 1 month of follow-up.Results Compared with preoperative levels,serum FSH,LH,E 2,PRL,P and T were not changed after follow-up for 1 month in observation group(P>0.05),but serum FSH,LH,PRL,P and T were increased and E 2 was decreased after follow-up for 1 month in control group(P<0.05).Compared with control group,serum levels of FSH,LH,PRL,P and T and incidence of complications were reduced but E 2 level was elevated in observation group after follow-up for 1 month(P<0.05).Conclusion Hysteroscopic endometrium electrosection has no obvious effect on ovarian function and is associated with low incidence of complications and high safety in the treatment of functional uterine bleeding.
