输卵管性不孕症采取经阴道声诺维超声造影与宫腹腔镜检查的诊断价值比较

摘　　要：目的讨论输卵管性不孕症采取经阴道声诺维超声造影与宫腹腔镜检查的诊断价值比较。方法随机抽取滨州医学院附属医院2013年6月—2015年8月收治的94例患者有188条输卵管作为诊断对象,分为对照组和实验组,每组47例,94条输卵管;对照组采取宫腹腔镜检查,实验组采取宫腹腔镜前经阴道声诺维超声造影,对比宫腹腔镜和超声造影检查结果。结果 94条输卵管中,声诺维超声造影检查,输卵管通畅:双侧通畅33条,单侧通畅22条,共55条输卵管;输卵管通而不畅:双侧通而不畅14条,单侧通而不畅10条,共24条输卵管,输卵管阻塞:双侧不通9条,单侧不通6条,共15条输卵管。宫腹腔镜检查结果显示,输卵管通畅42条,31条通而不畅,21条阻塞,声诺维超声造影诊断:敏感度87%,特异性93%,阳性预测值89%,阴性预测值86%。实验组阴道声诺维超声造影检查准确率高于对照组(P<0.05)。结论经阴道声诺维超声造影可清晰观察输卵管走形、形态,是否通畅,诊断准确率较高,是检查输卵管性不孕症准确性较高的检查方法。Objective To discuss the comparison of transvaginal sonvue contrast-enhanced sonography and laparoscopy in patients with tubal infertility.Methods 188 fallopian tubes of 94 cases of patients admitted and treated in Affiliated Hospital of Binzhou Medical College from June 2013 to August 2015 were randomly extracted and divided into two groups with 47 cases and 94 fallopian tubes in each,the control group used the laparoscopy guided hysteroscope examination,while the experimental group used the transvaginal sonoVue contrast-enhanced sonography on the basis of the control group,and the laparoscopy guided hysteroscope and ultrasonic contrast examination results were compared.Results Of 94 fallopian tubes,the transvaginal sonovue contrast-enhanced sonography showed that there were 33 bilateral patency,22 single patency,55 fallopian tubes in total,and 14 bilateral partially obstructed tubes,10 single partially obstructed tubes and 24 fallopian tubes in total,and there were 9 bilateral obstruction,6 single obstruction,15 fallopian tubes in total,and the laparoscopy guided hysteroscope results showed that there were 42 fallopian patency,31 cases partially obstructed tubes and 21 obstructed tubes,and the transvaginal sonvue contrast-enhanced sonography showed that the sensitivity,specificity,positive predicative value and negative predicative value were respectively 87%,93%and 89%,86%,and the accurate rate of transvaginal sonvue contrast-enhanced sonography in the experimental group was higher than that in the control group(P<0.05).Conclusion The transvaginal sonvue contrast-enhanced sonography can clearly observe the direction,shape and patency of fallopian tubes,with higher diagnostic accuracy,which is an examination method of tubal infertility with high accuracy.
