摘 要:目的研究血管内皮生长因子(VEGF)对急性心肌梗死(AMI)大鼠心功能和细胞凋亡的影响。方法采用结扎冠状动脉的方法建立大鼠急性心肌梗死模型,取40只模型大鼠,按照随机数字表法分为模型组20只和VEGF治疗组20只,并设假手术组20只,所有大鼠均尾静脉注射给药。4周后,通过高频率超声影像系统检测以下指标:收缩末期左室内径(LVIDs)、射血分数(EF)、舒张末期左室内径(LVIDd)、每搏输出量(SV)、短轴缩短率(FS)。生化分析法测定血清中心肌酶含量;TUNEL染色法观察心肌细胞凋亡状况,Western blotting法测定心肌组织caspase-3蛋白表达;RT-PCR法测定心肌组织bcl-2 mRNA、Bax mRNA表达。结果与模型组比较,经VEGF治疗能够显著降低急性心肌梗死大鼠LVIDd和LVIDs,提高FS、EF和SV;显著降低血清中心肌酶(AST、CPK、LDH)含量;明显改善肌组织细胞凋亡状况,显著降低心肌细胞凋亡指数(AI),显著下调caspase-3蛋白表达,显著上调bcl-2 mRNA表达并显著下调Bax mRNA表达,提高bcl-2/Bax mRNA表达比值,上述差异均具有统计学意义(P<0.05)。结论 VEGF具有改善AMI大鼠心功能的作用,并且可对AMI大鼠心肌细胞凋亡起到有效抑制作用,作用机制可能与VEGF可对凋亡相关基因、蛋白表达起到有效调节作用存在一定的相关性。Objective To investigate the effects of vascular endothelial growth factor(VEGF)on the cardiac function and cardiomyocytes apoptosis in the rats with acute myocardial infarction.Methods The model of acute myocardial infarction was established by means of ligation coronary artery,and 40 rats were randomly divided into model(20 rats)group and VEGF treatment group(20 rats)according to random digital table method,and the sham operation group and the tail vein were administered to all the rats.After 4 weeks,high frequency ultrasound imaging system was used to detect the following indicators:end systolic left interior diameter(LVIDs),ejection fraction(EF),end-diastolic diameter left indoor(LVIDd),each cardiac output(SV),short axial shortening rate(FS),biochemical analysis,the determination of serum myocardial enzyme levels;TUNEL staining method was used to observe the apoptosis of myocardial cells.Western blotting method was used to determine the expression of caspase-3 protein in myocardial tissue,and RT-PCR method was used to determine the expression of myocardial tissue bcl-2 mRNA and Bax mRNA.Results Compared with model control group,the LVIDd,LVIDs in VEGF treated group were significantly decreased,the FS,EF,SV were significantly increased;the content of AST,CPK,LDH in serum were significantly decreased,the cardiomyocyte apoptosis was significantly improved and the AI were significantly decreased,the expression of caspase-3 were examined by Western blotting,the expression of bcl-2 mRNA,Bax mRNA were determined,the ratio of bcl-2/Bax was significantly increased;all of the difference above were significant(P<0.05).Conclusion VEGF can improve cardiac function in AMI rats,and can effectively restrain myocardial apoptosis.The mechanism may be related to that,vascular endothelial growth factor could regulate the expression of apoptosis related gene and protein.
