摘　　要：目的探讨在超声引导下甲状腺结节针吸引活检标本中miRNA-146b的表达水平对术前诊断的影响。方法回顾性选取2015年1月至2016年12月在新疆医科大第二附属医院行细针穿刺细胞学检查(FNAB)的40例甲状腺结节患者。采用RT-PCR技术检测甲状腺结节FNAB样本中miRNA-146b的表达,比较甲状腺乳头状癌与甲状腺良性结节miRNA-146b表达差异。其结果与细胞学诊断与术后病理诊断进行比较。结果使用miRNA-146b的2^(-△△CT)值鉴别甲状腺乳头状癌和甲状腺良性结节,其诊断甲状腺乳头状癌和甲状腺良性结节的ROC曲线下面积为0.818,敏感度为80%,特异度为80%,显示miRNA表达诊断甲状腺良恶性结节有较高的效能。在与传统穿刺细胞病理学比对中敏感度100%比85%,优于传统方法,但敏感度一致均为100%。结论 miRNA-146b在甲状腺乳头状癌及甲状腺良性结节中的表达差异有统计学意义,即miRNA-146b表达可鉴别甲状腺良恶性结节,优于传统穿刺细胞病理学检查。Objective To discuss the influence of expression level of miRNA-146b in the thyroid nodule needle biopsy specimens on the preoperative diagnosis.Methods 40 patients with thyroid nodules who performed FNAB were selected in this study.RT-PCR was used to detect the miRNA-146b expression level.The miRNA-146b expression level in papillary thyroid carcinoma(PTC)and thyroid benign nodule samples were compared.The results of cytological diagnosis and postoperative pathological diagnosis were compared.Results When differential diagnosis of thyroid papillary carcinoma and thyroid benign nodules was performed by using the miRNA-146b 2-△△CT,the area under the ROC curve was 0.818,the sensitivity was 80%,specificity of 80%,which showed higher efficiency of miRNA expression.The sensitivity was 100%higher than the traditional method,but the sensitivity was 100%.Conclusion The miRNA-146 b expression level can be used in the differential diagnoses of benign and malignant thyroid nodules.
