摘 要:目的观察乌司他丁对大鼠肺缺血再灌注损伤的保护作用,并探讨乌司他丁对肺缺血再灌注损伤中肺组织Clara细胞分泌蛋白(CC16)的影响。方法将24只雄性SD大鼠使用随机数字表随机分为假手术组(Sham组,n=8)、缺血再灌注损伤组(I/R组,n=8)和乌司他丁组(ULI组,n=8)。I/R组使用止血钳夹闭大鼠左肺动静脉血管30min后松开止血钳,再灌注120 min后处死大鼠。Sham组大鼠不阻断肺血管,其他处理同I/R组。ULI组于再灌注开始时静脉给予ULI 0.5 ml(10 000 U/kg),其他处理同I/R组。检测三组大鼠动脉血氧分压(PaO_2)、二氧化碳分压(PaCO_2)和湿/干重比(W/D),测定肺组织丙二醛(MDA)和超氧化物歧化酶(SOD),采用ELISA方法测定肺泡灌洗液(BALF)中CC16蛋白含量,RT-PCR方法测定肺组织CC16 mRNA水平。结果与Sham组相比,I/R组和ULI组PaO_2、SOD、BALF中CC16含量和肺组织的mRNA表达水平均显著降低(P<0.05),PaCO_2、W/D值和MDA均显著升高(P<0.05);但是与I/R组相比,ULI组PaO_2、SOD、BALF中CC16含量和肺组织的mRNA表达水平均显著升高(P<0.05),PaCO_2、W/D值和MDA均显著降低(P<0.05)。结论乌司他丁可以缓解肺缺血再灌注导致的损伤和氧化应激水平,并且可以增加CC16蛋白的分泌和表达。Objective To observe the beneficial effect of ulinastatin(ULI)on lung tissue with ischemia and reperfusion injury,and to explore the involvement of CC16 during the process.Methods Twenty-four male SD rats were randomly divided into 3 groups by random number table:sham group,ischemia and reperfusion group(I/R group)and ulinastatin group(ULI group).In I/R group,the hemostatic forceps were used to close the left pulmonary arteriovenous shunt and loosened after 30min.After reperfusion for 120 min,all rats were sacrificed to detect arterial oxygen pressure(PaO 2),partial pressure of carbon dioxide(PaCO 2),wet/dry weight ratio(W/D)of lung tissue,levels of malondialdehyde(MDA)and superoxide dismutase(SOD)of lung tissue,CC16 protein content in bronchoalveolar lavage fluid(BALF)by using ELISA method,the level of CC16 mRNA of lung tissue by using RT-PCR method.The pulmonary vessels in rats of sham group did not block,and other treatment was the same as I/R group.In ULI group,ULI 0.5 ml(10 000 U/kg)was given through the vein at the beginning of reperfusion,and other treatment was the same as that of I/R group.Results In comparison with Sham group,PaO 2,SOD,CC16 content in BALF and the level of CC16 mRNA in lung tissue was significantly lower(P<0.05),PaCO 2,W/D and MDA were significantly increased(P<0.05)in the I/R group and ULI group;but in comparison with I/R group,PaO 2,SOD and CC16 content in BALF and the level of CC16 mRNA in lung tissue were significantly increased(P<0.05),PaCO 2,W/D and MDA were significantly lower in ULI group(P<0.05).Conclusion Ulinastatin may reduce the lung injury induced by ischemia and reperfusion,and increase the secretion and expression of CC16 protein.
