鱼骨图分析法在腔镜手术物品清点安全管理中的应用

摘　　要：目的探讨鱼骨图分析法在腔镜手术物品清点安全管理中的应用。方法选取2016年9月—12月妇科腹腔镜手术127例作为对照组,使用常规开放手术物品清点的方法;另外选取2017年1月—4月妇科腹腔镜手术130例作为观察组,采用鱼骨图分析后的综合改良方法,观察比较两组腔镜手术物品清点在术前、术中、术后的差错发生率。结果对照组的差错例数术前、术中、术后分别为11、16、13例,即差错发生率为8.66%、12.60%、10.24%;观察组的差错例数术前、术中、术后分别为2、3、2例,即差错发生率为1.54%、2.31%、1.54%;对照组的差错发生率高于观察组,差异有统计学意义(P<0.05)。结论鱼骨图分析法能够全面性、根本性分析腔镜手术物品清点在术前、术中、术后各个环节存在的安全隐患,及时采取针对性措施,可以降低腔镜手术物品清点差错率的发生,保证手术的顺利完成。Objective To explore the application of fishbone-digram-based counting in safety management in endoscopic operation.Methods We enrolled 127 cases of gynecological laparoscopic surgery from September to December 2016 as control group,where the conventional method was used to count open surgical items and another 130 cases of gynecological laparoscopic surgery as the observation group from January to April 2017,where the fishbone-diagram-based counting.The two groups were compared in terms of the error incidence of endoscopic surgical article counting during preoperation,intraoperation and postoperation.Results The number of errors of the control group were 11,16 and 13 at the three time points,respectively,with the error rate of 8.66%,12.60%and 10.24%,respectively.However.the number of errors in the observation group was 2,3 and 2,respectively,with the error rate of 1.54%,2.31%and 1.54%,respectively.The errors of the control group was higher than that of the observation group(P<0.05).Conclusion Fishbone-diagram-based counting is effective in the comprehensive and fundamental analysis of the risks of the endoscopic operation checking security at preoperation,intraoperation and postoperatioin by reducing the error rate of surgical article counting and ensuring the smooth completion of operation.
