配型血小板的疗效及临床应用

摘　　要：目的通过评价分析患者输注配型血小板的疗效,以进一步提高患者治疗的有效性。方法分析中山大学附属第一医院2016年患者输注配型血小板及随机血小板的治疗前后数据,配对比较分析患者输注配型血小板及随机血小板的治疗效果。结果 2016年全院患者输注血小板共5 874 U,申请输注配型血小板202人次,45例患者成功配型并多次输注配型血小板,输注前配型血小板组和随机血小板组两组血小板计数差异不具统计学意义(P>0.05),而输注后两组血小板计数差异具有统计学意义(P<0.05)。配型血小板组和随机血小板组的24 h CCI差异具有统计学意义(P<0.05),输注配型血小板后患者血小板计数升高较明显,出血情况明显改善。结论输注配型血小板对于血小板输注无效患者疗效显著,提倡需多次输注血小板患者检测血小板抗体及输注配型血小板,提升输注血小板治疗效果。Objective To analyze the curative effect of the platelet transfusion in the patients in order to further improve the treatment effectiveness.Methods We analyzed the data of matched platelet transfusion and random platelet transfusion in the First Affiliated Hospital of Sun Yat-sen University in 2016 and compared the curative effect of matched platelet transfusion and random platelet transfusion.Results In 2016,a total of 5 874 U of platelets were transfused in our hospital,202 times of matching platelets were applied for transfusion,45 patients were successfully matched and multiple times of matching platelets were transfused.There was no statistically significant difference in platelet count between matched platelet group and random platelet group before infusion(P>0.05),but there was statistically significant difference in platelet count between two groups after infusion(P<0.05).The 24-hour CCI difference between matched platelet group and random platelet group was statistically significant(P<0.05).Conclusion Transfusion of platelet matching for platelet transfusion invalid patients curative effect is obvious.We advocate the need for multiple transfusion of platelet patients to detect platelet antibodies and transfusion of platelet matching in order to improve the effect of platelet transfusion treatment.
