摘  要:目的 建立大鼠背根神经节(DRG)慢性压迫模型,探讨磷酸化P38(p-P38)在神经病理性疼痛中的作用机制。方法 将SD大鼠分为假手术组(sham组,切开皮肤与肌肉)、慢性背根神经节压迫组(CCD组,L_3与L_4背根神经节插入不锈钢棒)、治疗组(行CCD手术并注射SB203580)。SB203580分别于术前1d,术后1,7d鞘内注射。各组大鼠分别在术后1、3、5、7、14 d检测机械痛和热痛阈值。使用Western blotting检测DRG中p-P38表达情况。结果 与假手术组比较,CCD组术后p-P38水平增加(P<0.05);与CCD组比较,治疗组大鼠DRG中p-P38蛋白表达明显减少(P<0.05);与sham组比较,CCD组大鼠机械缩腿阈值明显降低(P<0.01);与CCD组比较,治疗组大鼠机械缩腿阈值明显升高(P<0.05)。与sham组比较,CCD组、治疗组大鼠对热刺激缩腿反射时间明显缩短(均P<0.01)。CCD组、治疗组大鼠比较热刺激缩腿反射时间没有统计学差异(P>0.05)。结论 大鼠外周神经损伤后p-P38表达升高,进而引发神经病理性疼痛。Objective To prove the hypothesis that chronic compression of the dorsal root ganglion produces allodynia through the phosphorylated P38(p-P38)pathway.Methods Sprague-Dawley rats were divided into three groups:sham-operated rats,chronic compression of dorsal root ganglion(CCD,achieved using surgery)rats without treatment,and CCD rats treated with the P38 inhibitor,SB203580.Rats in the treatment group were administered SB203580 intrathecally starting 1 day before or 1 day or 7 days after CCD surgery.All rats were sacrificed at 14 days after surgery to examine the p-P38 activity in the DRG using western blot analysis.Mechanical and heat allodynia were tested at 1,3,5,7,and 14 days after surgery.Results p-P38 levels were significantly higher in the CCD group than in the sham-operated group(P<0.05).p-P38 levels were significantly lower(P<0.05)in the treatment group than in the CCD group.The mechanical withdrawal threshold(MWT)was significantly lower in the CCD group than in the sham-operated group(P<0.01);the MWT was significantly higher in the treatment group than in the CCD group(P<0.05).Heat withdrawal latency(HWL)was significantly lower in the CCD and treatment groups than in the sham-operated group(P<0.01).There was no significant difference in HWL between the CCD and treatment groups.Conclusion p-P38 expression of rat DRG,which increased after CCD,results in neuropathic pain.
