探讨不同固定方式在深静脉置管中的应用价值

摘　　要：目的研究不同固定方式在深静脉置管中的应用价值。方法方便选取该院2013年1月—2016年4月收治的深静脉置管患者900例为对象进行分组。其中翼固定组均为2014年10月之前病例,采用固定翼固定;直接局部缝线固定组均为2014年10月(含当月)之后病例,采用无翼直接局部缝线方法固定。比较两组患者穿刺点感染率、导管脱出发生率;患者舒适度评分、疼痛评分、深静脉置管时间。结果无翼直接局部缝线组跟翼固定组比较,前者穿刺点7、14、21 d感染率分别为2.22%、4.44%、6.67%,而后者穿刺点7、14、21 d感染率分别为11.11%、22.22%、24.44%;前者导管总脱出发生率为2.22%,后者导管总脱出率为13.33%(P<0.05);无翼直接局部缝线固定组与翼固定组患者舒适度评分分别为(9.51±1.41)分、(8.39±1.77)分,而疼痛评分分别为(1.51±0.21)分、(3.39±0.57)分,深静脉置管时间分别为(45.62±3.21)d、(32.61±1.59)d(P<0.05)。结论不同固定方式在深静脉置管中的应用均有一定价值,但无翼直接局部缝线方法固效果优于用翼固定,可有效提高固定的稳定性,减少导管脱出发生率,减少局部感染的发生,有利于提升患者舒适度,减轻痛苦,延长置管时间,值得推广。Objective To research the application value of different fixation methods in the deep venous catheterization.Methods 900 cases of patients with deep venous catheterization in our hospital from January 2013 to April 2016 were convenient selected and divided into the wing fixation group before October 2014 and direct local suturing fixation group after October 2014(including the same month),and the puncture point infection rate,incidence rate of catheter extrusion,comfort degree score,pain score and deep venous catheterization time were compared between the two groups.Results The puncture point infection rates in 7,14d and 21 d in the no-wing direct local suturing group and in the wing fixation group were respectively 2.22%,4.44%,6.67%and 11.11%,22.22%,24.44%,and the total incidence rate of catheter extrusion was respectively 2.22%and 13.33%(P<0.05),and the comfort degree,pain score and deep venous catheterization time in the no-wing direct local suturing fixation group and in the wing fixation group was respectively(9.51±1.41)points,(1.51±0.21)points,(45.62±3.21)d and(8.39±1.77)points,(3.39±0.57)points,(32.61±1.59)d(P<0.05).Conclusion The application of different fixation methods in the deep venous catheterization has a certain value,but the effect of no-wing direct local suturing method is better than that of wing fixation,which can effectively improve the fixation stability,reduce the incidence rates of catheter extrusion and local infection,and it is conducive to improving the comfort degree and relieving pains and prolonging the catheterization time,and it is worth promotion.
