摘 要:目的了解过度动态气道塌陷(EDAC)在慢阻肺患者中的发病情况,评价EDAC对慢阻肺患者的肺功能、急性加重、运动能力的影响。方法 141例慢阻肺患者进入研究,通过纤支镜检查诊断有无EDAC。合并EDAC的慢阻肺患者为EDAC组,在无合并EDAC的慢阻肺患者中随机选取20例为单纯慢阻肺组。比较两组肺通气功能、弥散功能、六分钟步行距离、近一年急性加重次数。结果 141例慢阻肺患者中合并EDAC者18人,发病率12.77%。两组的肺通气功能、弥散功能、近一年急性加重次数均无统计学显著差异(p均>0.05)。EDAC组的分钟通气量、六分钟步行距离显著低于单纯慢阻肺组,差异有统计学显著意义(P<0.05)。结论在部分慢阻肺患者中合并存在过度动态气道塌陷。过度动态气道塌陷对慢阻肺患者肺通气功能无影响,但可使分钟通气量及运动能力下降。Objective To explore the prevalence of excessive dynamic airway collapse in patients with COPD,and to evaluate the effect of excessive dynamic airway collapse(EDAC)on lung function,acute exacerbation and exercise capacity of patients with COPD.Methods 141 COPD patients was investigated.All patients were examined with bronchoscopy to see whether complicated with EDAC or not.The patients with COPD complicated with EDAC were assigned to the EDAC group.20 COPD patients were selected randomly from the rest patients as the control group.Their lung function(spirometry and diffusion),acute exacerbation in one year and the six-minute walking distance were compared between the two groups.Results Among 141 patients with COPD,18 patients showed EDAC,and the overall incidence was 12.77%(18/141).Among 18 cases,there were 13 cases with a collapse of 50-75%,and 5 cases with a collapse of≥75%.There was no statistically significant difference in FVC,FEV 1,FVC%pred,FEV 1%pred,DLCO%pred and acute exacerbation between the two groups(P>0.05).Compared with the control group,the six-minute walking distance and maximum ventilatory volume(MVV%pred)were lower in the EDAC group(P<0.05).Conclusion There are a fair number of COPD patients complicated with EDAC.EDAC has no effect on lung function,but it can influence the maximum ventilatory volume and exercise capacity of patients with COPD.
