摘  要:目的探讨纤维环缝合对兔腰椎间盘髓核部分摘除后MR信号的影响。方法选用新西兰白兔42只分为实验组及对照组,每组各21只,每组再设术后2、4、12周三个亚组,每个亚组7只。实验组(缝合组)于L2-3椎体前缘纵形切开纤维环,针刺使髓核突出,然后缝合纤维环;对照组(未缝合组)未给予缝合纤维环直接逐层闭合手术切口。术后2、4、12周两组L2-3间盘分别行MR T2加权像扫描并进行Pfirrmann分级。实验组术后12周处死并解剖动物检查纤维环缝合的可靠性。结果术后2、4、8周开始实验组L2-3间盘的MR T2加权像扫描信号明显高于对照组,统计学分析差异有统计学意义(P<0.05),Pfirrmann分级对比差异有统计学意义(P<0.05)。结论纤维环缝合可减缓髓核摘除术后椎间盘退变进程。Objective To investigate the effect of anular suture on the MR signal and the height of intervertebral space after partial extirpation of lumbar intervertebral disc in rabbits.Methods 42 New Zealand white rabbits were divided into study group and control group,each group with postoperative 2,4,12 weeks subgroups,7 rats in each subgroup.The study group(suture group)longitudinally cut the anular on the anterior edge of the L2-3 vertebral body,the needle made the nucleus pulposus protruding,and then sutured the anular.The control group(not sutured group)did not suture the annulus,close the incision directly.2,4,12 weeks after operation L2-3 disc were performed MR T2 scan and Pfirrmann classification were measured in both groups.Results Postoperative 2,4,8 weeks,the MR T2WI signal of L2-3 disc in the study group was significantly higher than that in the control group(P<0.05).There were also differences in Pfirrmann grading(P<0.05).Conclusion Annular suture can slow down the degeneration of intervertebral disc after nucleus pulposus extirpation.
