摘 要:目的探讨"球囊挤压"技术在冠状动脉慢性完全闭塞病变(CTO)中应用的可行性和有效性。方法选取平顶山市第二人民医院心内科于2013年4月~2017年5月期间行经皮冠状动脉介入治疗(PCI)的闭塞处存在分支血管的CTO患者41例,其中男性26例,女性15例,随机分为治疗组(19例)和对照组(22例),治疗组采用"球囊挤压"技术行PCI,对照组采用常规方法治疗。比较两组间一般临床资料、X线曝光时间、对比剂用量、PCI失败率、手术并发症、1年内心血管主要不良事件等情况。结果两组患者一般临床资料及冠状动脉造影特点比较差异无统计学意义。两组间各指标比较,X线曝光时间对照组为(42.4±8.2)min,治疗组为(33.6±4.9)min;对比剂用量对照组(196.0±11.6)ml,治疗组(174.8±15.6)ml;PCI失败率对照组6例(27.3%),治疗组1例(5.3%),两组比较差异均有统计学意义(P均<0.05)。比较两组手术并发症,对照组1例(4.5%),治疗组2例(10.5%);比较两组主要心血管事件,对照组1例(4.5%),治疗组3例(15.8%),比较均无统计学差异(P均>0.05)。结论"球囊挤压"术可以降低CTO病变介入术的失败率、减少X线曝光时间及对比剂用量,未明显增加手术并发症及主要心血管事件。"球囊挤压"术在CTO病变介入术中应用有效、可行。Objective To investigate the feasibility and effectiveness of“Balloon Compression”technology in the treatment of chronic total occlusions(CTO).Methods 41 patients with CTO lesions admitted from April 2013 to May 2017 were enrolled and randomly divided into treatment group(n=19)and conventional group(n=22).General clinical data,radiation exposure,material consumption,the failure rates of percutaneous coronary intervention(PCI),complications and major adverse cardiac events were compared between two groups.Results There was no significant difference of the general clinical characteristics and angiographic features between two groups(P>0.05).Compared to conventional group,radiation exposure[(33.6±4.9)min vs.(42.4±8.2)min],material consumption[(174.8±15.6)ml vs.(196.0±11.6)ml]were fewer in treatment group(P<0.05).The failure rates of PCI[1(5.3%)vs.6(27.3%)]was lower in treatment group(P<0.05).There was no significant difference of complications[2(10.5%)vs.1(4.5%)]and major adverse cardiac events[3(15.8%)vs.1(4.5%)]between two groups(P>0.05).Conclusions“Balloon Compression”technology have lowered failure rates of PCI,lower radiation exposure and material consumption in the treatment of CTO lesions,without increasing complications and major adverse cardiac events significantly.“Balloon Compression”technology is efficient and feasible in PCI of CTO.
