辛伐他汀治疗腹膜透析患者营养不良的作用及机制研究

摘　　要：目的探讨辛伐他汀对长期腹膜透析患者营养不良状况的作用。方法将148例长期腹膜透析患者按随机数字表法分为试验组和对照组,试验组71例接受辛伐他汀治疗,对照组77例接受安慰剂治疗。检测治疗前后超敏C反应蛋白(hs-CRP),白细胞介素-6(IL-6),白细胞介素-10(IL-10)的表达水平,测量治疗前后身体质量指数(BMI)、上臂围、上臂肌围和肱三头肌皮褶厚度(TSF),检测血液中总蛋白、白蛋白、前白蛋白、红细胞、血红蛋白、血细胞比容、铁蛋白、转铁蛋白、未饱和铁、总铁结合力等指标。结果试验组辛伐他汀治疗8周后降低了hs-CRP、IL-6水平,升高了IL-10水平,患者BMI、上臂围、上臂肌围和TSF水平均上升,总蛋白、白蛋白、前白蛋白、红细胞、血红蛋白、血细胞比容、铁蛋白、转铁蛋白、未饱和铁、总铁结合力等指标均较治疗前改善,且优于对照组(P<0.05)。结论辛伐他汀通过减轻腹膜透析患者微炎症水平,改善其营养不良状况。To investigate whether simvastatin could improve the malnutrition level of patients with peritoneal dialysis.One hundred and forty-eight patients with long-term peritoneal dialysis were randomly divided into observation and control groups.The observation group(n=71)received simvastatin treatment while the control group(n=77)received placebo treatment.Levels of hs-CRP,IL-6 and IL-10 were detected before and after treatments.The indexes of malnutrition such as body mass index,mid-arm circumference,mid-arm circumference,mid-arm muscle circumference and TSF were measured.Serum total protein,albumin,prealbumin,red blood cells,hemoglobin,haematocrit,ferritin,transferring,unsaturated iron and total iron binding capacity were also measured.Simvastatin reduced the levels of hs-CRP and IL-6 while increased IL-10 level after 8 weeks of treatment.Body mass index,mid-arm circumference,mid-arm circumference,mid-arm muscle circumference and TSF levels in the observation group were higher than those in the control group.The parameters of serum total protein,albumin,prealbumin,red blood cells,hemoglobin,haematocrit,ferritin,transferring,unsaturated iron and total iron binding capacity were all improved when compared to before treatment.Simvastatin can improve malnutrition of patients with peritoneal dialysis by reduction of the inflammation level.
