以血管紧张素Ⅱ受体拮抗剂为基础的单片复方制剂对高血压患者治疗预后的影响

摘　　要：目的评估使用以血管紧张素Ⅱ受体拮抗剂(ARB)为基础的2种不同单片复方降压制剂治疗对高血压患者预后影响。方法 113例高血压病患者,分别采用厄贝沙坦/氢氯噻嗪片150 mg/12.5 mg qd与缬沙坦/氨氯平地片80 mg/5 mg qd治疗,比较两组治疗2年后的血压达标率、新发心血管预后不良事件发生率(脑梗死、心功能衰竭,肾功能衰竭)。结果两组患者血压达标率、肾功能衰竭事件发生率比较,差异无统计学意义(P>0.05),厄贝沙坦/氢氯噻嗪组心功能衰竭事件发生率低于缬沙坦/氨氯平地片组,差异有统计学意义(P<0.05),脑梗死事件发生率高于缬沙坦/氨氯平地片组,差异有统计学意义(P<0.05)。结论不同单片复方制剂均能安全有效地控制血压,改善患者的依从性,且应根据患者不同基础靶器官受损情况个体化选用不同的复方降压制剂。To evaluate the effect of different combined antihypertensive drugs based on angiotensin II receptor antagonist(ARB)on the prognosis of hypertension patients.One hundred and thirteen hypertension patients were treated with the combined valsartan/amlodpipine(80 mg/5 mg qd)group and the combined irbesartan/hctz(150 mg/12.5 mg qd)group.The blood pressure target achievement and the incidence of the primary cardiovascular events such as heart failure,renal failure and stroke were observed after 2 years.No statistical significance was observed for the rate of blood pressure target achievement between the two groups(P>0.05).There was also no significant difference in the incidence of renal failure between the two groups(P>0.05).However,the incidence of heart failure in the combined valsartan/amlodpipine group was lower than the combined irbesartan/hctz group(P<0.05).The incidence of stroke in the combined irbesartan/hctz group was higher than the combined valsartan/amlodpipine group(P<0.05).The patients with hypertension should be managed with reasonable individualized treatment according to different target organ damage.
