糖尿病足合并抑郁状态中医证型分布规律的研究

摘　　要：目的调查并对比糖尿病足(DF)患者抑郁的患病率,探究DF合并抑郁患者中医证型的分布规律。方法选取2015年3月—11月就诊的DF患者171例为DF组,以及糖尿病(DM)患者120例为DM组。比较两组抑郁的患病率,对其中抑郁单因子得分≥2分的71例DF患者进行中医辨证和足部辨证。结果 DF组抑郁患病率高于DM组(χ~2=6.036,P=0.014)。DF合并抑郁患者的中医辨证分型中肝气郁结型所占比例和抑郁因子得分最高,而足部辨证分型中热盛肉腐型所占比例和抑郁因子得分最高。肝气郁结型DF合并抑郁患者足部辨证分型中热盛肉腐型所占比例也是最高。结论 DF患者易发生抑郁的消极情绪。DF合并抑郁患者的主要证型为肝气郁结,足部辨证的主要证型为热盛肉腐,肝气郁结与热盛肉腐可相互影响。Objective To investigate and compare incidence rate of depression in patients with diabetic foot(DF),and to study distribution rule of traditional Chinese medicine(TCM)syndromes in patients with DF combined with depression.Methods A total of 171 DF patients admitted during March 2015 and November were selected as DF group,and other 120 patients with diabetes mellitus(DM)at the same period were selected as DM group.Incidence rate of depression was compared in two groups,and then 71 DF patients with depression scores equal or more than 2 points were analyzed by TCM syndrome differentiation and foot syndrome differentiation.Results Incidence rate of depression in DF group was significantly higher than that in DM group(χ2=6.036,P=0.014).In DF patients combined with depression,proportion of liver-qi stagnation type and depression factor score by TCM syndrome differentiation were the highest,while proportion of Resheng Roufu type and depression factor score by syndrome differentiation were the highest.Proportion of Resheng Roufu type in DF patients with liver-qi stagnation type combined with depression was the highest by foot syndrome differentiation.Conclusion DF patients easily have depression and negative emotions.The main syndromes of DF patients combined with depression are liver-qi stagnation type in TCM syndrome differentiation and Resheng Roufu type in foot syndrome differentiation,and liver-qi stagnation and Resheng Roufu can influence each other.
