摘 要:目的观察脾虚痰湿证代谢综合征(metabolic syndrome,MS)患者肠道主要菌群差异。方法选取2015年1月—5月就诊的MS患者80例,其中脾虚痰湿证及气阴两虚证各40例,并选取同期健康体检者30例作为健康对照组。提取粪便基因组DNA,应用16S rRNA V3序列设计各菌属特异性引物,以质粒作为标准品,对各目标菌属进行绝对定量PCR分析。结果脾虚痰湿证组肠道总菌群、双歧杆菌、乳酸杆菌数量显著低于健康对照组,肠球菌属、大肠杆菌、球形梭菌、柔嫩梭菌数量显著高于健康对照组(P<0.05)。脾虚痰湿证组双歧杆菌数量显著低于气阴两虚证组,大肠杆菌数量显著高于气阴两虚证组(P<0.05)。气阴两虚证组总菌群数量低于健康对照组,肠球菌属数量高于健康对照组(P<0.05)。结论 MS患者存在肠道菌群失调情况,不同中医证型间肠道菌群存在差异,以脾虚痰湿证患者尤为明显,菌群改变可能对脾虚痰湿证的形成起重要作用,中医健脾利湿法可能通过调节肠道菌群而达到治疗MS的目的。Objective To observe differences of mainly enteric microbiota in patients with metabolic syndrome(MS)of phlegm-dampness due to spleen deficiency.Methods A total of 80 MS patients from January to May 2015 included phlegm-dampness due to spleen deficiency(n=40)and Qi-yin asthenia syndrome(n=40),and other 30 healthy people taking medical examination at same period were selected as control group.Genomic DNA samples were extracted from feces,and 16S rRNA V3 sequences was used to design every bacterial-specific primer,and then plasmid was used as a proof sample to perform absolutely quantitative polymerase chain reaction(PCR)analysis for every target organism.Results In phlegm-dampness due to spleen deficiency group,numbers of totally intestinal flora,bifidobacterium and lactobacillus were significantly less,while numbers of enterococcus,escherichia coli,clostridium sporogenes and clostridium difficile were significantly more than those in control group(P<0.05).In phlegm-dampness due to spleen deficiency group,bifidobacterium number was significantly less,while number of escherichia coli was significantly more than those in Qi-yin asthenia syndrome group(P<0.05).In Qi-yin asthenia syndrome group,number of totally intestinal flora was significantly less,while enterococcus number was significantly more than those in control group(P<0.05).Conclusion MS patients have alteration of intestinal flora conditions,and differences can be found in intestinal flora among different TCM syndrome types,and patients with phlegm-dampness due to spleen deficiency are more obvious.Bacterial flora changes may have an important role in formation of phlegm-dampness due to spleen deficiency,and TCM method of strengthening spleen and eliminating dampness may treat MS by regulating intestinal flora.
