五种中药注射剂联合ARB或ACEI治疗早期糖尿病肾病的网状meta分析

摘　　要：目的采用网状meta分析比较5种中药注射剂联合血管紧张素受体拮抗剂或血管紧张素转换酶抑制剂(ARB或ACEI)治疗早期糖尿病肾病(DN)的疗效。方法全面检索多种数据库,获取5种中药注射剂联合ARB或ACEI治疗早期DN的临床随机对照试验,筛选和纳入研究后进行质量评价,数据采用R、GeMTC、STATA软件进行分析。结果共纳入45个临床随机对照试验。网状meta分析显示,黄芪注射液联合组降低尿蛋白排泄率效果优于其他4组;仅舒血宁注射液联合组降低尿素氮效果显著;丹红和灯盏花素注射液联合组降低血肌酐效果显著,且丹红注射液联合组>灯盏花素注射液联合组;5种注射液组降低糖化血红蛋白效果均不理想;丹红、黄芪和舒血宁注射液联合组降低总胆固醇效果显著,且舒血宁注射液联合组>黄芪注射液联合组>丹红注射液联合组;舒血宁、黄芪注射液联合组降低甘油三酯效果显著,且舒血宁注射液联合组>黄芪注射液联合组。结论黄芪注射液降低尿蛋白排泄率效果较好,舒血宁注射液降低尿素氮、总胆固醇、甘油三酯效果较好,丹红注射液降低血肌酐效果较为理想。Objective To investigate curative effects of five kinds of Chinese herb injections(CHIs)combined with angiotensin receptor antagonist or angiotensin converting enzyme inhibitors(ARB or ACEI)in treatment of patients with diabetic nephropathy(DN)during early period by network Meta-analysis.Methods Randomized controlled trials(RCTs)about 5 kinds of Chinese herb injections combined with ARB or ACEI in treatment of DN patients during early period were searched overall from databases.RCTs were performed quality evaluation after being screened and recruited,and related data was analyzed by R,GeMTC and STATA softwares.Results A total of 45 RCTs were finally included.Network Meta-analysis showed that effect of decreasing urinary protein excretion rate by Mongolian Milkvetch Root combination group were better than those by other 4 kinds of CHI groups;only Shuxuening injection combination group could achieve significantly effect on decreasing urea nitrogen;Danhong and Breviscapine injections combination groups could achieve significantly effects on decreasing serum creatinine level,and the decreasing level in Danhong injection combination group was significantly larger than that in Breviscapine injection combination group.Effects of decreasing glycosylated hemoglobin levels were poor by 5 kinds of CHI groups.Danhong,Mongolian Milkvetch Root and Shuxuening injection combination groups could effectively decrease total cholesterol levels,and the decreasing levels from high to low were Shuxuening,Mongolian Milkvetch Root and Danhong injection combination groups.Shuxuening and Mongolian Milkvetch Root injection combination groups could effectively decrease triglyceride levels,and the effect by Shuxuening injection combination group was better than that by Mongolian Milkvetch Root injection combination group.Conclusion Mongolian Milkvetch Root injection can achieve good effect on decreasing urinary protein excretion rate,and Shuxuening injection can achieve good effect on decreasing blood urea nitrogen,total cholesterol and trigl
