经筋排刺法治疗肘管综合征临床疗效观察

摘　　要：目的:观察经筋排刺法对肘管综合征的临床疗效及对尺神经电生理的影响;方法:选取符合诊断及纳入标准的肘管综合征(CTS)患者56例,随机分为对照组(28例)和治疗组(28例)。其中对照组给予甲钴胺片口服治疗,治疗组给予经筋排刺法治疗,两组患者治疗期间均给予夜间肘关节支具制动,疗程3周,分别于治疗前后观察两组患者尺神经功能评分及神经电生理的改变,并评价其临床疗效。结果:治疗组治疗后总有效率为96.4%,明显优于对照组(P<0.05);与本组治疗前比较,治疗组疗程结束后尺神经功能评分显著升高(P<0.05),肘上-肘下、肘-腕的MCV及指V-腕、指V-肘的SCV显著增快(P<0.01),肘上、肘下运动神经的波幅显著增高(P<0.01),治疗后组间比较,差异均具有显著统计学差异(P<0.01);结论:经筋排刺法能够显著恢复CTS患者的肢体功能,减轻尺神经的卡压及尺神经的损伤,临床疗效显著。Objective:To observe the clinical effect of Jingjin aligning needling treating cubital tunnel syndrome(CTS)and its influence to nerve electrophysiology.Methods:56 CTS patients were randomly divided into the control group(n=28)and the treatment group(n=28).The control group was treated with Mecobalamin tablets and the treatment group was given Jingjin aligning needling.The patients in both groups were made immobilization with elbow brace at night during the treatment.The course of the treatment was three weeks.Score of ulnar nerve function and nerve electrophysiology were observed and compared before and after the treatment in the two groups to assess the clinical effect.Results:The total effective rate was 96.4%in the treatment group,which was significantly higher than 85.7%in the control group(P<0.05).The score of ulnar nerve function was significantly increased after the treatment compared to that before the treatment in the treatment group(P<0.05);In the treatment group,MCV of elbow-wrist and MCV of above elbow-below elbow,as well as SCV of finger V-wrist and of finger V-elbow were significantly faster after treatment(P<0.01);and the motor nerve amplitudes above elbow and below elbow were obviously increased(P<0.01);of which there were statistical differences between the two groups(P<0.01).Conclusion:Jingjin aligning needling can obviously restore the limbs function of CTS patients,and it can alleviate the compression and the injury of ulnar nerve,with a remarkable clinical effect.
